| 1 | A national report from China Liver Transplant Registry: steroid avoidance after liver transplantation for hepatocellular carcinoma显示文摘Objective:We aimed to evaluate the efficacy and safety of steroid-free immunosuppression after liver transplantation(LT)for hepatocellular carcinoma(HCC).Methods:We retrospectively analyzed HCC recipients without steroids after LT(SF group,n=368)based on the China Liver Transplant Registry(CLTR)database.These recipients were matched 1:2 with patients using steroids(S group,n=736)for the same period after LT for HCC,according to propensity scores.Results:Multivariate analysis indicates that recipients with younger age[odds ratio(OR),1.053;P=0.011],preoperative hepatitis B virus(HBV)DNA≥1,000 copies/mL(OR,2.597;P=0.004)and beyond Milan criteria(OR,4.255;P<0.001)were identified as the risk factors associated with tumor recurrence in steroid avoidance recipients after LT.The patients fulfilling the Milan criteria in the SF group presented higher overall and tumor-free survival rates than those in the S group(P<0.05).Multivariate analysis revealed that recipient beyond Milan criteria was an independent prognostic factor for overall survival(OR,1.690;P<0.001)and tumor-free survival(OR,2.066;P<0.001).The incidences of new-onset diabetes mellitus(21.20% vs.33.29%,P<0.001),new-onset hypertension(10.05%vs.18.61%,P<0.001)and hyperlipidemia(4.08% vs.7.20%,P=0.042)were significantly lower in the SF group.Conclusions:Steroid-free immunosuppression could be safe and feasible for HBV-related HCC patients in LT.Age,HBV DNA level and Milan criteria maybe risk factors associated with tumor recurrence in steroid avoidance recipients.Recipient beyond Milan criteria was an independent prognostic factor and recipient fulfilling Milan criteria can benefit the most from steroid-free immunosuppression. | Qiang Wei Feng Gao Runzhou Zhuang Qi Ling Qinghong Ke Jian Wu Tian Shen Mangli Zhang Min Zhang Xiao Xu Shusen Zheng | 2017 | Chinese Journal of Cancer Research2017,29,5: | 25 |
| 2 | Personalized treatment based on mini patient-derived xenografts and WES/RNA sequencing in a patient with metastatic duodenal adenocarcinoma显示文摘Background:Treatment guidelines for a variety of cancers have been increasingly used in clinical practice,and have resulted in major improvement in patient outcomes.However,recommended regimens(even first-line treat-ments)are clearly not ideal for every patients.In the present study,we used mini patient-derived xenograft(mini-PDX)and next-generation sequencing to develop personalized treatment in a patient with metastatic duodenal adenocarcinoma.Methods:Resected metachronous metastatic tumor tissues were implanted into SCID mice to determine the sensitivity to a variety of drug regimens.Mutation profiles were assessed using both DNA whole-exome sequencing(DNA-WES)and RNA sequencing.The results of the analyses were used to select optimal treatment for the patient with metastatic duodenal adenocarcinoma.Results:Assessment with mini-PDX models took only 7 days.The results showed high sensitivity to S-1 plus cis-platin,gemcitabine plus cisplatin and everolimus alone.The patient received gemcitabine plus cisplatin initially,but the treatment was terminated due to toxicity.The patient was then switched to treatment with S-1 alone.The overall disease-free survival was 34 months.DNA-WES and RNA sequencing identified KRAS mutation(A146T),TP53(C229Yfs*10)and RICTOR amplification in the metastatic duodenal adenocarcinoma.These findings provided further support to the results of the mini-PDX,and suggest mTOR inhibitors should be used if and when relapse eventually occurs in this patient.Conclusions:Mini-PDX model combined with WES/RNA sequencing can rapidly assess drug sensitivity in cancer patients and reveal key genetic alterations.Further research on this technology for personalized therapy in patients with refractory malignant tumors is warranted. | Peng Zhao Hui Chen Danyi Wen Shuo Mou Feifei Zhang Shusen Zheng | 2018 | Cancer Communications2018,38,1: | 11 |
| 3 | Stereotactic body radiation therapy versus radiofrequency ablation in patients with small hepatocellular carcinoma:a systematic review and meta-analysis显示文摘Background:This study aimed to compare the clinical outcomes and toxicity between small hepatocellular carcinoma(HCC)patients treated with stereotactic body radiation therapy(SBRT)and those treated with radiofrequency ablation(RFA).Methods:We searched databases for relevant clinical studies.The primary outcomes of interest were overall survival(OS)at 1 and 2 years,freedom from local progression(FFLP)rate at 2 years,and complications.Results:Five cohorts from 5 retrospective studies and 4,814 patients with HCC were included.Pooled OS at 2 years was significantly lower for SBRT than for RFA[odds ratio(OR):0.63;95%confidence interval(CI):0.51-0.79;P<0.0001],but the pooled FFLP rate at 2 years was higher for SBRT than for RFA(OR:1.66;95%CI:1.05-2.61;P=0.03).In addition,there was no significant difference in the local and liver toxicities of the two treatments.The contradictory conclusion between the OS and FFLP outcome may be attributed to the difference in radiological dose and location,but there were no uniform criteria to illustrate the radiological dose and location in the included studies.Conclusions:SBRT had a higher local control ratio but poorer prognosis than RFA in patients with small HCC.The local toxicity was comparable in both treatments.Further trials should be designed with uniform standards for SBRT and RFA treatments. | Jiawei Hong Linping Cao Haiyang Xie Yuanxing Liu Jun Yu Shusen Zheng | 2021 | Hepatobiliary Surgery and Nutrition2021,10,5: | 5 |

| 5 | Structural shifts in the intestinal microbiota of rats treated with cyclosporine A after orthotropic liver transplantation显示文摘Understanding the effect of immunosuppressive agents on intestinal microbiota is important to reduce the mortality and morbidity from orthotopic liver transplantation (OLT). We investigated the relationship between the commonly used immunosuppressive agent cyclosporine A (CSA) and the intestinal microbial variation in an OLT model. The rat samples were divided as follows:(1) N group (normal control);(2) I group (isograft LT, Brown Norway [BN] rat to BN);(3) R group (allograft LT, Lewis to BN rat);and (4) CSA group (R group treated with CSA). The intestinal microbiota was assayed by denaturing gradient gel electrophoresis profiles and by using real-time polymerase chain reaction. The liver histopathology and the alanine/aspartate aminotransferase ratio after LT were both ameliorated by CSA. In the CSA group, the numbers of rDNA gene copies of Clostridium cluster I, Clostridium cluster XIV, and Enterobacteriaceae decreased, whereas those of Faecalibacterium prausnitzii increased compared with the R group. Cluster analysis indicated that the samples from the N, I, and CSA groups were clustered, whereas the other clusters contained the samples from the R group. | 6 | Single-center Experience in the Diagnosis and Treatment of Hepatic Perivascular Epithelioid Cell Neoplasm显示文摘Background and Aims:Perivascular epithelioid cell neo-plasms(PEComas)are a rare type of mesenchymal neo-plasm and their preoperative diagnosis is challenging.In this study,we summarized the experience from a single medical center to study the examinations,clinical presen-tations,and pathological and histological characteristics of PEComas in the liver in order to optimize overall un-derstanding of the diagnosis and treatment of these neo-plasms.Methods:We conducted a retrospective analysis to investigate the clinical and pathological characteristics as well as imaging presentations of 75 patients diagnosed with hepatic PEComa in The First Affiliated Hospital of Zhe-jiang University between April 2010 and April 2020.Re-sults:Among the 75 patients,52 were women,and the median age was 48 years.Most patients had no specific symptoms,and two were admitted to the hospital for a second time owing to relapse.All patients underwent surgi-cal resection.Histologically,38 patients had classical angio-myolipoma(AML)and 37 had epithelioid AML.The PECo-mas were accompanied by positive immunohistochemical expression of HMB45,Melan-A,and smooth muscle actin.Follow-up data were obtained from 47 of the total 75 pa-tients,through October 2020.Two patients had metastasis after surgery.Conclusions:AML is the most common type of hepatic PEComa.There are no specific symptoms of he-patic PEComa,and serological examinations and imaging modalities for accurate preoperative diagnosis are lacking.Epithelioid AML should be considered a tumor of uncertain malignant potential;however,the prognosis of PEComa af-ter resection is promising. | Junjun Jia Jia Luo Cheng-Gen Pan Guomei Ge Meibao Feng Bei Zou Li Liu Shusen Zheng Jun Yu | 2022 | Journal of Clinical and Translational Hepatology2022,10,1: | 5 |

| 8 | Severity of early allograft dysfunction following donation after circulatory death liver transplantation:a multicentre study显示文摘Background:Early allograft dysfunction(EAD)is associated with decreased graft and patient survival rates.This study aimed to identify the severity of EAD and develop a predictive model for EAD after donation after circulatory death(DCD)liver transplantation(LT).Furthermore,the influence of operative time on EAD incidence was also evaluated.Methods:In this retrospective,multicentre cohort study,nomograms were established based on a single-centre training cohort(n=321)and validated in a 3-center validation cohort(n=501).Results:The incidence rate of EAD was 46.4%(149/321)in the training cohort and 40.5%(203/501)in the validation cohort.Of the 149 EAD patients in the training cohort,77 patients with either elevated alanine aminotransferase(ALT)or aspartate aminotransferase(AST)were classified as having EAD type A,and the rest of the EAD patients were classified as having EAD type B.Recipients with EAD type B had lower graft and patient survival rates than recipients with EAD type A(P=0.043 and 0.044,respectively).We further developed a nomogram to predict EAD(graft weight,cold ischemia time,donor age,model for end-stage liver disease(MELD)score)and another nomogram to predict EAD type B(graft weight,cold ischemia time,MELD score).The nomograms for the prediction of EAD and EAD type B had good discrimination[concordance index(C-index)=0.712(0.666-0.758),0.707(0.641-0.773)]and calibration[Hosmer-Lemeshow(HL)P=0.384,P=0.425]in the validation cohort.An increased operative time(>6 h)was associated with increased EAD and EAD type B incidence in the high-risk group(P=0.005,P=0.020,respectively).Conclusions:EAD type B was associated with decreased graft and patient survival rates.The novel nomograms effectively predicted the incidence of EAD and EAD type B in DCD LT patients. | Kun Wang Di Lu Yuhui Liu Wangyao Li Li Zhuang Zhenyu Ma Qinfen Xie Binhua Pan Yichao Wu Junli Chen Lidan Lin Xiaowen Feng Qiang Wei Xuyong Wei Haiyang Xie Zhengxin Wang Shusen Zheng Xiao Xu | 2021 | Hepatobiliary Surgery and Nutrition2021,10,1: | 4 |

| 11 | Clear mortality gap caused by graft macrosteatosis in Chinese patients after cadaveric liver transplantation显示文摘Background:Liver transplantation(LT)is one of the most effective surgical treatment for patients with end-stage liver disease.Steatosis is a contributor for inferior graft quality.But its impact and safety on transplantation was less assessed in Chinese patients.Methods:Graft steatosis and related information involved in recipients,donors and surgical procedures were retrospectively collected from 239 patients.Results:Donor macrosteatosis(MaS)caused about 2.14 and 2.80 folds of increment on patient and graft mortality.Dose-response analysis revealed prominent risk of grafts on overall patient/organ mortality when MaS content exceeded 10%(P<0.05).Noteworthy,deaths were only observed in MaS group when concurrent with extremely higher post-transplant alanine aminotransferase(ALT,64%).However,microsteatosis(MiS)grafts didn’t affect outcomes after LT.In a cohort of Chinese patients,MaS had comprehensive effects on post-transplant outcomes with relatively lower safety threshold at 10%.Mortality gap caused by MaS grafts was observed in patients with severer ischemia reperfusion injury.Conclusions:Our study revealled the graft MaS affected the post-transplant outcomes in lower risk cutoff in Chinese patients.Further study is worthy to validate these results and investigate inner mechanism under the phenomenon. | Zhengtao Liu Wenchao Wang Li Zhuang Jingfeng Liu Shuping Que Dan Zhu Linfang Dong Jian Yu Lin Zhou Shusen Zheng | 2020 | Hepatobiliary Surgery and Nutrition2020,9,6: | 2 |
| 12 | The circFASN/miR-33a pathway participates in tacrolimusinduced dysregulation of hepatic triglyceride homeostasis显示文摘Dyslipidemia exhibits a high incidence after liver transplantation,in which tacrolimus,a widely used immunosuppressant,plays a fundamental role.MicroRNAs and related circRNAs represent a class of noncoding RNAs that have been recognized as important regulators of genes associated with lipid metabolism.However,their transcriptional activities and functional mechanisms in tacrolimus-related dyslipidemia remain unclear.In this study,we observed that tacrolimus could induce triglyceride accumulation in hepatocytes by stimulating sterol response element-binding proteins(SREBPs)and miR-33a.Our in silico and experimental analyses identified miR-33a as a direct target of circFASN.Tacrolimus could downregulate circFASN and result in elevated miR-33a in vivo and in vitro.Overexpression of circFASN or silencing of miR-33a decreased the promoting effects of tacrolimus on triglyceride accumulation.Clinically,the incidence of dyslipidemia in liver transplant recipients with elevated serum miR-33a after liver transplantation was higher than that in patients without elevated serum miR-33a(46.3%vs.18.8%p=0.012,n=73).Our results showed that the circFASN/miR-33a regulatory system plays a distinct role in tacrolimus-induced disruption of lipid homeostasis.MiR-33a is likely a risk factor for tacrolimus-related dyslipidemia,providing a potential therapeutic target to combat tacrolimus-induced dyslipidemia after liver transplantation. | Chenzhi Zhang Kangchen Chen Rongli Wei Guanghan Fan Xuechun Cai Li Xu Beini Cen Jianguo Wang Haiyang Xie Shusen Zheng Xiao Xu | 2020 | Signal Transduction and Targeted Therapy2020,5,1: | 2 |
