1Segmental intrahepatic cholestasis as a technical complication of the transjugular intrahepatic porto-systemic shunt显示文摘BACKGROUND Segmental intrahepatic cholestasis caused by transjugular intrahepatic portosystemic shunt(TIPS)(SIC-T),is a rare complication of this technique and only referred by case reports.Thus,we conducted a systematic,retrospective analysis to provide evidence regarding prevalence and consequences of this TIPS-induced bile duct compression.AIM To assess prevalence and outcome of SIC-T in a large TIPS-cohort.METHODS In this retrospective cohort study,we screened the institutional databases for all consecutive patients that were treated by TIPS-placement or TIPS-revision between January 2005 and August 2013.We analyzed radiologic images for signs of biliary congestion.Cases that were indicative of SIC-T were reviewed by two independent radiologists and additional patient data was collected.Descriptive statistics of patient demographics,indications for TIPS and procedural details were registered.Logistic regression analysis was performed to identify predictors for the development of SIC-T.RESULTS We analyzed 135 cirrhotic patients who underwent TIPS(mean age 55 years,79%male gender).Etiology of cirrhosis was alcohol in most cases and indications for TIPS were mainly refractory ascites and recurrent variceal bleeding.TIPS revision was necessary in 31 patients.We identified 4 cases(2.9%)of SIC-T in direct proximity of the TIPS-stent.Diagnosis was confirmed by CT-scan,MRI or endoscopic retrograde cholangio pancreaticography(ERCP).In two patients TIPS was implanted via the right and in one through the medial hepatic vein.One patient received TIPS-prolongation by multiple revisions.Most patients were asymptomatic but one cholangitic abscess necessitated a transhepatic drain.Logistic regression analysis identified TIPS-placement other than from medial hepatic vein to right portal vein as risk factor(OR 21.0)for SIC-T.CONCLUSION SIC-T ads to(mostly late)complications in the interventional treatment of cirrhotic portal hypertensions and can lead to cholangitic abscesses.Patients,particularly with multiple interventions,should be screened for SIC-T.Julian Nikolaus Bucher Marcus Hollenbach Steffen Strocka Gereon Gaebelein Michael Moche Thorsten Kaiser Michael Bartels Albrecht Hoffmeister 2019World Journal of Gastroenterology2019,25,43:2
2An inverted triplesalen ligand by a convergent synthesis and its influence on trinuclear Fe_3~Ⅲ complexes显示文摘The inverted triplesalen ligand H 6 feld Me has been synthesized from 2,4,6-triformyl-phloroglucinol and a ketimine salen half-unit in a convergent synthesis. NMR, IR, and UV-vis spectroscopy reveal that this ligand is not in the O-protonated tautomer but in the N-protonated tautomer with substantial heteroradialene contribution. This ligand and the conventional triplesalen ligand H 6 talen t-Bu 2 have been used to synthesize the trinuclear Fe III complexes [(feld Me )(FeCl) 3 ] and [(talen t-Bu 2 )(FeCl) 3 ], respectively. The molecular structures of these complexes were obtained by single-crystal X-ray diffraction. Two trinuclear Fe III complexes of [(feld Me )(FeCl) 3 ] dimerize via two Fe-phenolate bonds, whereas due to steric hindrance no dimerization is observed for [(talen t-Bu 2 )(FeCl) 3 ]. The structural data also reveal some heteroradialene contribution in the trinuclear complexes. Whereas UV-vis and M bauer spectroscopy are not suitable to distinguish between the two complexes, FT-IR spectra show characteristic features due to the different substitution patterns of the conventional and the inverted triplesalen ligands. Another handle is provided by electrochemistry. Whereas both complexes exhibit an irreversible oxidation wave (0.94 V vs. Fc + /Fc for [(feld Me )(FeCl) 3 ] and 0.84 V vs. Fc + /Fc for [(talen t-Bu 2 )(FeCl) 3 ]), which is assigned to the oxidation of the central backbone, higher potential oxidations are reversible for [(talen t-Bu 2 )(FeCl) 3 ]) but irreversible for [(feld Me )(FeCl) 3 ]. This is attributed to the reversible oxidation of the terminal phenolates in the di-tert-butyl substituted [(talen t-Bu 2 )(FeCl) 3 ] in contrast to the mono-methyl-substituted phenolates in [(feld Me )(FeCl) 3 ]. The magnetic properties of [(talen t-Bu 2 )(FeCl) 3 ] reveal a very small ferromagnetic coupling with significant zero-field splitting of the FeⅢ S = 5/2 ions. 5Off-label-use of sulfur-hexafluoride in voiding urosonography for diagnosis of vesicoureteral reflux in children:A survey on adverse events显示文摘AIM To evaluate the risk profile of sulfur hexafluoride in voiding urosonography(VUS) based on a large cohort of children.METHODS Since 2011 sulfur hexafluoride(SH,SonoV ue?,Bracco,Italy) is the only ultrasound contrast available in the European Union and its use in children has not been approved.Within a 4-year-period,531 children with suspected or proven vesicoureteral reflux(f/m = 478/53; mean age 4.9 years; 1 mo-25.2 years) following parental informed consent underwent VUS with administration of 2.6 ± 1.2 mL SH in a two-center study.A standardizedtelephone survey on adverse events was conducted three days later.RESULTS No acute adverse reactions were observed.The survey revealed subacute,mostly self-limited adverse events in 4.1%(22/531).The majority of observed adverse events(17/22) was not suspected to be caused by an allergic reaction: Five were related to catheter placement,three to reactivated urinary tract infections,five were associated with perineal disinfection before voiding urosonography or perineal dermatitis and four with a common cold.In five patients(0.9%) hints to a potential allergic cause were noted: Perineal urticaria was reported in three interviews and isolated,mild fever in two.These were minor self-limited adverse events with a subacute onset and no hospital admittance was necessary.Ninety-six point two percent of the parents would prefer future VUS examinations with use of SH.CONCLUSION No severe adverse events were observed and indications of self-limited minor allergic reactions related to intravesical administration of SH were reported in less than 1%.Alexander Sauer Clemens Wirth Isabel Platzer Henning Neubauer Simon Veldhoen Alexander Dierks Reinhard Kaiser Andreas Kunz Meinrad Beer Thorsten Bley 2017World Journal of Clinical Pediatrics2017,6,1:1
