| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. It accounts for more than 90% of all esophageal squamous cell carcinoma cases in China. Geographical differentiation is one of the most distinctive characteristics of esophageal cancer. The progression, risk factors, and prognosis of these two subtypes of esophageal cancer differ. Based on the mining depth and thickness of the coal seam, roadway support technology solutions with different buried depth and thickness of coal seam are proposed. Support schemes are amended and optimized in time through monitoring data of the deformation of roadway,roof separation, I-beam bracket, bolt and anchor cable force to ensure the long-term stability and security of the roadway surrounding rock and support structure. | 6 | Precise nanomedicine for intelligent therapy of cancer显示文摘Precise nanomedicine has been extensively explored for efficient cancer imaging and targeted cancer therapy, as evidenced by a few breakthroughs in their preclinical and clinical explorations. Here, we demonstrate the recent advances of intelligent cancer nanomedicine, and discuss the comprehensive understanding of their structure-function relationship for smart and efficient cancer nanomedicine including various imaging and therapeutic applications, as well as nanotoxicity. In particular, a few emerging strategies that have advanced cancer nanomedicine are also highlighted as the emerging focus such as tumor imprisonment, supramolecular chemotherapy, and DNA nanorobot. The challenge and outlook of some scientific and engineering issues are also discussed in future development. | 7 | Identification and differentiation of Panax ginseng,Panax quinquefolium, and Panax notoginseng by monitoring multiple diagnostic chemical markers显示文摘To differentiate traditional Chinese medicines(TCM) derived from congeneric species in TCM compound preparations is usually challenging. The roots of Panax ginseng(PG), Panax quinquefolium(PQ) and Panax notoginseng(PN) are used as popular TCM. They contain similar triterpenoid saponins(ginsenosides) as the major bioactive constituents. Thus far, only a few chemical markers have been discovered to differentiate these three species. Herein we present a multiple marker detection approach to effectively differentiate the three Panax species, and to identify them in compound preparations. Firstly, 85 batches of crude drug samples(including 32 PG, 30 PQ, and 23 PN) were analyzed by monitoring 40 major ginsenosides in the extracted ion chromatograms(EICs) using a validated LC–MS fingerprinting method. Secondly, the samples were clustered into different groups by pattern recognition chemometric approaches using PLS-DA and OPLS-DA models, and17 diagnostic chemical markers were discovered. Aside from the previously known Rf and p-F11, ginsenoside Rs1 could be a new marker to differentiate PG from PQ. | 8 | Clinical features of nonalcoholic fatty liver disease-associated hepatocellular carcinoma显示文摘BACKGROUND: Nonalcoholic fatty liver disease (NAFLD), especially nonalcoholic steatohepatitis, is a recognized risk factor for hepatocellular carcinoma (HCC). However, detailed analysis of the clinical features in patients with NAFLD and their association with HCC is lacking. This study aimed to update the clinical features of patients with NAFLD-associated HCC. DATA SOURCES: The clinical data of patients with NAFLD- associated HCC from 25 studies published between 1990 and 2010 in the Pubmed database were comprehensively reviewed. RESULTS: In a total of 169 patients with NAFLD-associated HCC, 72.8% were male. The median age at abnormal liver function tests and diagnosis of NAFLD and HCC was 60, 64 and 67 years, respectively. Most patients were obese (75%) and diabetic (59.8%), 32.3% had dyslipidemia, and 53% had hypertension. Nearly all patients (98.6%, 71/72) were complicated with at least one metabolic disorder. The majority (76%) of the HCC patients had a solitary tumor nodule, with the tumor size ranging from 0.8 to 20 cm in diameter (mean 3.4 cm). Most (61.1%) of the patients had moderately-differentiated HCC. In 40.2% of the patients, HCC occurred in the absence of cirrhosis. Among 130 patients, 57.7% underwent hepatectomy and 14.6% received liver transplantation. The mean follow-up of the treated patients for 25 months showed that 32.4% (24/74) died and 18.8% (9/48) had recurrence. CONCLUSIONS: Patients with NAFLD-associated HCC are usually accompanied with metabolic disorders. | 9 | The prognostic and therapeutic implications of circulating tumor cell phenotype detection based on epithelial-mesenchymal transition markers in the first-line chemotherapy of HER2-negative metastatic breast cancer显示文摘Background:Epithelial-mesenchymal transition(EMT)is implicated in the metastatic process and presents a chal-lenge to epithelial cell adhesion molecule-based detection of circulating tumor cells(CTCs),which have been demon-strated to be a prognostic indicator in metastatic breast cancer.Although evidence has indicated that heterogeneity of CTCs based on EMT markers is associated with disease progression,no standard recommendations have been established for clinical practice.This study aimed to evaluate the prognostic significance of dynamic CTC detection based on EMT for metastatic breast cancer patients.Methods:We enrolled 108 human epidermal growth factor receptor 2-negative metastatic breast cancer patients from the prospective phase III CAMELLIA study and applied the CanPatrol CTC enrichment technique to identify CTC phenotypes(including epithelial CTCs,biphenotypic epithelial/mesenchymal CTCs,and mesenchymal CTCs)in peripheral blood samples.Receiver operating characteristic curve analyses of total CTC count and the proportion of mesenchymal CTCs for predicting the 1-year progression-free survival(PFS)rate were conducted to determine the optimal cut-off values,and Kaplan-Meier analysis and Cox proportional hazards regression analysis were performed to investigate the prognostic value of the cut-off values of both total CTC count and the proportion of mesenchymal CTCs in combination.Results:For predicting the 1-year PFS rate,the optimal cut-off value of total CTC count was 9.5(Area under the curve[AUC]=0.538,95%confidence interval[CI]=0.418-0.657),and that of the proportion of mesenchymal CTCs was 10.7%(AUC=0.581,95%CI=0.463-0.699).We used the two cut-off values in combination to forecast PFS in which the total CTC count was equaled to or exceeded 10/5 mL with the proportion of mesenchymal CTCs surpassed 10.7%.Patients who met the combined criteria had significantly shorter median PFS than did those who did not meet the criteria(6.2 vs.9.9 months,P=0.010).A nomogram was constructed based on the criteria and significant clinicopatho-logical characteristics with a C-index of 0.613(P=0.010).Conclusions: The criteria, which combine the total CTC count and the proportion of mesenchymal CTCs, may be used to monitor therapeutic resistance and predict prognosis in patients with metastatic breast cancer. | Xiuwen Guan Fei Ma Chunxiao Li Shiyang Wu Shangying Hu Jiefen Huang Xiaoying Sun Jiayu Wang Yang Luo Ruigang Cai Ying Fan Qiao Li Shanshan Chen Pin Zhang Qing Li Binghe Xu | 2019 | Cancer Communications2019,39,1: | 15 |

After two rounds of panning with gastric carcinomacell line AGS highly expressing MC3-binding antigen, thephage clones displaying ScFv fragments of the antibodywere selected by ELISA. 4 phage clones showing strongsignal in ELISA were used to infect E. coil HB2151 toexpress soluble ScFvs. The soluble ScFve were identified byDot blot and Western blot, and their antigen-binding activitywas assayed by ELISA. The VH and VL DNAs of the ScFvDNA derived from phage clone 19 were sequenced.RESULTS: The VH, VL and ScFv DNAs were about 340 bp,320 bp and 750 bp respectively. After two rounds of panningto the recombinant phages, 18 antigen-positive phageclones were selected from 30 preselected phage clones byELISA. All the soluble ScFvs derived from the 4 out of the 18antigen-positive phage clones were about Mr 32 000 andconcentrated in periplasmatic space under the given culturecondition. The soluble ScFvs could bind the antigen, andthey shared the same binding site with MC3. | 11 | Commonly used animal models of non-alcoholic steatohepatitis显示文摘BACKGROUND:Animal models are an essential tool in non-alcoholic steatohepatitis (NASH) studies. Ideally, such models should reflect the etiology, disease progression, and the established pathology of human NASH. To date, no single animal model displays the range of histopathologic and pathophysiologic features associated with human NASH. The currently available models do not or only partially reflect the real picture of human NASH. In particular, insulin resistance and fibrosing steatohepatitis are rarely reproduced by the currently available models. Consequently, it is necessary to establish NASH models that can best mimic the real etiology, disease progression, and pathogenesis of human NASH. DATA SOURCES:We reviewed the major currently available animal models published in the literature (PubMed) and briefly commented on the pros and cons of these models. RESULT:Three major categories of animal models, genetic, dietary, and combination models, were reviewed and discussed. CONCLUSIONS:Animal models are not only useful in revealing the etiology of NASH, but also are important platforms for the assessment of therapeutic strategies. Currently available models do not reflect the full picture of NASH in patients. | 13 | Study on the Rapid Method to Predict Longevity of Controlled Release Fertilizer Coated by Water Soluble Resin显示文摘The study discussed the rapid method to test and predict the longevity of controlled release fertilizers (CRFs) coated by water soluble resin by using the short-term leaching under higher temperature. Pure water dissolving incubation and higher temperature leaching were used to study the patterns of the nutrient release of the CRFs. The correlation analysis between the days at 25°C and the hours at 80°C of Trincote 1 and Trincote 2 for the same cumulative release rates were conducted. Patterns of cumulative nutrient release curve followed one factor quadratic regression equation at each given temperatures,and each of relative coefficient was bigger than 0.995. As the temperature increased,nutrients release of the CRFs increased. The longevity of resin coated CRFs were predicted by use of both the cumulative nutrients release equation at 80°C and the regression equation of release time needed for the same cumulative release rates between 25 and 80°C. There were only 0.3-6.9% relative errors between the tested longevity and predicted one. In conclusion,the longevity of resin coated CRFs could be predicted more quickly and precisely by use of the higher temperature short-term leaching method than that of the traditional differential release rate. | 16 | Establishing a human pancreatic stem cell line and transplanting induced pancreatic islets to reverse experimental diabetes in rats显示文摘The major obstacle in using pancreatic islet transplantation to cure type I and some type II diabetes is the shortage of the donors. One of ways to overcome such obstacle is to isolate and clone pancreatic stem cells as 'seed cells' and induce their differentiation into functional islets as an abundant trans-plantation source. In this study, a monoclonal human pancreatic stem cell (mhPSC) line was obtained from abortive fetal pancreatic tissues. Pancreatic tissues were taken from abortive fetus by sterile procedures, and digested into single cells and cell clusters with 0.1% type IV collagenase. Cultured in modified glucose-low DMEM with 10% fetal bovine serum (FBS), these single cells and cell clusters adhered to culture dishes, and then primary epidermal-like pancreatic stem cells started to clone. After digesting with 0.25% trypsin and 0.04% EDTA, fibroblasts and other cells were gradually eliminated and epithelioid pancreatic stem cells were gradually purified during generations. Using clone-ring selection, the mhPSCs were obtained. After addition of 10 ng/mL epidermal growth factor (EGF) in cell culture medium, the mhPSCs quickly grew and formed a gravelstone-like monolayer. Continuously proliferated, a mhPSC line, which was derived from a male abortive fetus of 4 months old, has been passed through 50 generations. More than 1×109 mhPSCs were cryo-preserved in liquid nitrogen. Karyotype analysis showed that the chromosome set of the mhPSC line was normal diploid. Immunocytochemistry results demonstrated that the mhPSC line was positive for the pdx1, glucagon, nestin and CK19, and negative for the insulin, CD34, CD44 and CD45 protein expression. RT-PCR revealed further that the mhPSCs expressed transcription factors of the pdx1, glucagon, nestin and CK19. Also, in vitro induced with β-mercaptoethanol, the mhPSCs differentiated into nerve cells that expressed the NF protein. Induced with nicotinamide, the mhPSCs differentiated into functional islet-like clusters, as identified by di-thizone staining, which expressed the transcription factor of the insulin and secreted the insulin and C-peptide. | 18 | Risk factors for surgical-site infections after radical gastrectomy for gastric cancer:a study in China显示文摘Background:About 10%of patients get a surgical-site infection(SSI)after radical gastrectomy for gastric cancer,but SSI remains controversial among surgeons.The aim of this study was to explore the risk factors for SSIs after radical gastrectomy in patients with gastric cancer to guide clinical therapies and reduce the incidence of SSI.Methods:The study was a retrospective cohort study in patients who underwent radical gastrectomy for gastric cancer.SSI was defined in accordance with the National Nosocomial Infection Surveillance System.We evaluated patient-related and peri-operative variables that could be risk factors for SSIs.The Chi-squared test and logistic regression analysis were used to assess the association between these risk factors and SSI.Results:Among the 590 patients,386 were men and 204 were women.The mean age was 56.6(28-82)years and 14.2%(84/590)of these patients had an SSI.Among them,incisional SSI was observed in 23 patients(3.9%)and organ/space SSI in 61 patients(10.3%).Multivariate logistic regression analysis identified sex(odds ratios[ORs]=2.548,and 95%confidence interval[CI]:1.268-5.122,P=0.009),total gastrectomy(OR=2.327,95%CI:1.352-4.004,P=0.002),albumin level(day 3 after surgery)<30 g/L(OR=1.868,95%CI:1.066-3.274,P=0.029),and post-operative total parenteral nutrition(OR=2.318,95%CI:1.026-5.237,P=0.043)as independent risk factors for SSI.Conclusions:SSI was common among patients after radical gastrectomy for gastric cancer.The method supporting post-operative nutrition and the duration of prophylactic antibiotics may be important modifiable influencing factors for SSI. | Yong-Qi Qiao Li Zheng Bei Jia Wei-Hua Wang Xiao-Hao Zheng Lin-Lin Fan Yi-Bin Xie Yan-Tao Tian | 2020 | Chinese Medical Journal2020,,13: | 9 |
| 19 | Is Chinese Medicine Injection Applicable for Treating Acute Lung Injury and Acute Respiratory Distress Syndrome?A Systematic Review and Meta-analysis of Randomized Controlled Trials显示文摘Objective To assess the efficacy and safety of Chinese medicine injection(CMI)for treating acute lung injury/acute respiratory distress syndrome(ALI/ARDS).Methods Randomized controlled trials(RCTs)were identified by searching 3 English databases and 4 Chinese databases from their inceptions until February 2019.The Cochrane Handbook was used to evaluate risk of bias in the included studies.Data analysis was conducted using RevMan 5.3.3 software.Results A total of 19 eligible RCTs involving 1,334 participants was included in this systematic review and meta-analysis.The main meta-analysis showed that CMI combined with conventional therapy(CT)was more effective than CT alone in reducing the acute physiology and chronic health evaluation(APACHE)H score[mean difference(MD):−1.74 points,95%confidence interval(CI):−2.77 to−0.71,I^2=0]and increasing the total effective rate[relative risk(RR):1.35,95%CI:1.17 to 1.56,I^2=37%].Compared with CT,CMI combined with CT showed improvements in the arterial partial pressure of oxygen(PaO2,MD:9.25 mm Hg,95%CI:0.87 to 17.63,I^2=98%)and oxygenation index[arterial partial pressure of oxygen(PaO2)/fraction of inspired oxygen(FiO2),MD:50.75 mm Hg,95%CI:35.18 to 66.31,I^2=94%].CMI plus CT was superior to CT in reducing the systemic inflammatory response syndrome(SIRS)score(MD:−0.84 points,95%CI:−1.26 to−0.42,I^2=65%),length of hospital stay(MD:−4.22 days,95%CI:−6.49 to−1.95,I^2=92%),and duration of mechanical ventilation(MD:−2.94 days,95%CI:−4.68 to−1.21,I^2=89%).Only 1 study reported adverse events.Conclusions CMI as an adjuvant therapy showed great potential benefits for the treatment of ALI/ARDS.However,we could not make a definite conclusion due to low quality of included studies and uncertain security.Future studies should focus on improving research design,especially in blindness and placebo.The reporting of adverse events was also needed. | CHEN Yuan-bin LIU Qiao XIE Han YIN Shuo-miao WU Lei YU Xu-hua FAN Long LIN Lin | 2020 | Chinese Journal of Integrative Medicine2020,26,11: | 9 |
| 20 | Numerical analysis of broadband noise reduction with wavy leading edge显示文摘Large Eddy Simulation(LES) is performed to investigate the airfoil broadband noise reduction with wavy leading edge under anisotropic incoming turbulence. The anisotropic incoming turbulence is generated by a rod with a diameter of 10 mm. The incoming flow velocity is 40 m/s and the corresponding Reynolds numbers based on airfoil chord and rod diameter are about 397000 and 26000, respectively. The far-field acoustic field is predicted using an acoustic analogy method which has been validated by the experiment. A straight leading edge airfoil and a wavy leading edge airfoil are simulated. The results show that wavy leading edge increases the airfoil lift and drag whereas the lift and drag fluctuations are substantially reduced. In addition, wavy leading edge can significantly change the flow pattern around the leading edge and a pair of counter-rotating streamwise vortices stemming from each wavy leading edge peak are observed.An averaged noise reduction of 9.5 dB is observed with the wavy leading edge at the azimuthal angle of 90°. Moreover, the wavy leading edge can mitigate noise radiation at all the azimuthal angles without significantly changing the noise directivity.
