| 1 | Plasma exchange-centered artificial liver support system in hepatitis B virus-related acute-onchronic liver failure:a nationwide prospective multicenter study in China显示文摘BACKGROUND:Plasma exchange(PE)-centered artificial liver support system reduced the high mortality rate of hepatitis B virus(HBV)-related acute-on-chronic liver failure(ACLF).But the data were diverse in different medical centers.The present prospective nationwide study was to evaluate the effects of PE on patients with HBV-ACLF at different stages.METHODS:From December 2009 to December 2011,we evaluated 250 patients at different stages of HBV-ACLF from 10 major medical centers in China.All the laboratory parameters were collected at admission,before and after PE.RESULTS:Among the 250 patients who underwent 661 rounds of PE,one-month survival rate was 61.6%; 141(56.4%) showed improvement after PE.Variables such as age(P=0.000),levels of total bilirubin(TB,P=0.000),direct bilirubin(P=0.000),total triglycerides(P=0.000),low-density lipoprotein(P=0.022),Na+(P=0.014),Cl–(P=0.038),creatinine(Cr,P=0.007),fibrinogen(P=0.000),prothrombin time(PT,P=0.000),white blood cell(P=0.000),platelet(P=0.003) and MELD(P=0.000) were significantly related to prognosis.Multivariate logistic regression analysis showed that age,disease stage,TB,Cr and PT levels were independent risk factors of mortality among HBV-ACLF patients.CONCLUSIONS:PE can improve the clinical outcome of patients with HBV-ACLF.Levels of TB,Cr and PT,age and disease stage help to predict prognosis. | Jia-Jia Chen Jian-Rong Huang Qian Yang Xiao-Wei Xu Xiao-Li Liu Shao-Rui Hao Hui-Fen Wang Tao Han Jing Zhang Jian-He Gan Zhi-Liang Gao Yu-Ming Wang Shu-Mei Lin Qing Xie Chen Pan Lan-Juan Li | 2016 | Hepatobiliary & Pancreatic Diseases International2016,15,3: | 48 |

| 4 | The Association of Maternal Body Composition and Dietary Intake with the Risk of Gestational Diabetes Mellitus during the Second Trimester in a Cohort of Chinese Pregnant Women显示文摘Objective To investigate the association of maternal body composition and dietary intake with the risk of gestational diabetes mellitus(GDM). Methods A total 154 GDM subjects and 981 controls were enrolled in a prospective cohort study in 11 hospitals from May 20, 2012 to December 31, 2013. Bioelectrical impedance analysis and dietary surveys were used to determine body composition and to evaluate the intake of nutrients in subjects at 21-24 weeks' gestation(WG). Logistic regression analysis was applied to explore the relationships of maternal body composition and dietary intake with the risk of GDM morbidity. Results Age, pre-pregnant body weight(BW), and body mass index(BMI) were associated with increased risk of GDM. Fat mass(FM), fat mass percentage(FMP), extracellular water(ECW), BMI, BW, energy, protein, fat, and carbohydrates at 21-24 WG were associated with an increased risk of GDM. In contrast, fat free mass(FFM), muscular mass(MM), and intracellular water(ICW) were associated with a decreased risk of GDM. | 15 | Three-year results of small incision lenticule extraction and wavefront-guided femtosecond laser-assisted laser in situ keratomileusis for correction of high myopia and myopic astigmatism显示文摘AIM: To compare and calculate the 3-year refractive results, higher-order aberrations(HOAs), contrast sensitivity(CS) and dry eye parameters after small incision lenticule extraction(SMILE) and wavefront-guided femtosecond laser-assisted laser in situ keratomileusis(FS-LASIK) for correction of high myopia and myopic astigmatism.METHODS: In this prospective, non-randomized comparative study, 78 eyes with spherical equivalent(SE) of-8.11±1.09 diopters(D) received a SMILE surgery, and 65 eyes with SE of-8.05±1.12 D received a wavefront-guided FS-LASIK surgery with the Visu Max femtosecond laser(Carl Zeiss Meditec, Jena, Germany) for flap cutting. Visual acuity, manifest refraction, CS, HOAs, ocular surface disease index(OSDI) and tear break-up time(TBUT) were evaluated during a 3-year follow-up.RESULTS: The difference of uncorrected distance visual acuity(UDVA) postoperatively was achieved at 1 mo and at3 mo, whereas the difference of the mean UDVA between two groups at 3 y were not statistically significant(t=-1.59,P=0.13). The postoperative change of SE was 0.89 D in the FS-LASIK group(t=5.76, P=0.00), and 0.14 D in the SMILE group(t=0.54, P=0.59) from 1 mo to 3 y after surgery.At 3-year postoperatively, both HOAs and spherical aberrations in the SMILE group were obviously less than those in the FS-LASIK group(P=0.00), but the coma root mean square(RMS) was higher in the SMILE group(0.59±0.26) than in the FS-LASIK group(0.29±0.14, P=0.00).The mesopic CS values between two groups were not statistically significant at 3 y postoperatively. Comparedwith the FS-LASIK group, lower OSDI scores and longer TBUT values were found in the SMILE group at 1 mo and 3 mo postoperatively. With regard to safety, no eye lost any line of CDVA in both groups at 3 y after surgery. CONCLUSION: Both SMILE and wavefront-guided FSLASIK procedures provide good visual outcomes. | 16 | Human induced dryland degradation in Ordos Plateau,China,revealed by multilevel statistical modeling of normalized difference vegetation index and rainfall time-series显示文摘Land degradation causes serious environmental problems in many regions of the world,and although it can be effectively assessed and monitored using a time series of rainfall and a normalized difference vegetation index(NDVI)from remotely-sensed imagery,dividing human-induced land degradation from vegetation dynamics due to climate change is not a trivial task.This paper presented a multilevel statistical modeling of the NDVI-rainfall relationship to detect human-induced land degradation at local and landscape scales in the Ordos Plateau of Inner Mongolia,China,and recognized that anthropogenic activities result in either positive(land restoration and re-vegetation)or negative(degradation)trends.Linear regressions were used to assess the accuracy of the multilevel statistical model.The results show that:(1)land restoration was the dominant process in the Ordos Plateau between 1998 and 2012;(2)the effect of the statistical removal of precipitation revealed areas of human-induced land degradation and improvement,the latter reflecting successful restoration projects and changes in land management in many parts of the Ordos;(3)compared to a simple linear regression,multilevel statistical modeling could be used to analyze the relationship between the NDVI and rainfall and improve the accuracy of detecting the effect of human activities.Additional factors should be included when analyzing the NDVI-rainfall relationship and detecting human-induced loss of vegetation cover in drylands to improve the accuracy of the approach and eliminate some observed non-significant residual trends. | Jing ZHANG JianMing NIU Tongliga BAO Alexander BUYANTUYEV Qing ZHANG JianJun DONG XueFeng ZHANG | 2014 | Journal of Arid Land2014,6,2: | 16 |
| 17 | DEC1 nuclear expression:A marker of differentiation grade in hepatocellular carcinoma显示文摘AIM: To investigate the expression patterns of human differentiated embryo chondrocyte 1 (DEC1) in hepatocellular carcinoma (HCC) and corresponding adjacent non-tumor and the normal liver tissues, the association between DEC1 expression and histopathological variables and the role of DEC1 in hepatocarcinogenesis. METHODS: The expression of DEC1 was detected immunohistochemically in 176 paraffin-embedded sections from 63 patients with HCC and 50 subjects with normal liver tissues. RESULTS: DEC1 protein was persistently expressed in the cytoplasm of hepatocytes in normal liver and HCC tissues. Compared with adjacent non-tumor liver tissues, HCC tissues showed high nuclear expression of DEC1 protein. However, high DEC1 nuclear expression was more frequently detected in well-differentiated (83.3%) than in moderately (27.3%) and poorly differentiated HCC (16.7%). Low DEC1 expression was associated with poor histological differentiation and malignancy progression. A correlation was found between the nuclear expression of DEC1 protein and histological differentiation (r = 0.376, P = 0.024).
