1Contrast-enhanced ultrasonography assessment of gastric cancer response to neoadjuvant chemotherapy显示文摘AIM:To quantitatively assess the ability of double contrast-enhanced ultrasound(DCUS) to detect tumor early response to pre-operative chemotherapy.METHODS:Forty-three patients with gastric cancer treated with neoadjuvant chemotherapy followed by curative resection between September 2011 and February 2012 were analyzed.Pre-operative chemotherapy regimens of fluorouracil + oxaliplatin or S-1 + oxaliplatin were administered in 2-4 cycles over 6-12 wk periods.All patients underwent contrast-enhanced computed tomography(CT) scan and DCUS before and after two courses of pre-operative chemotherapy.The therapeutic response was assessed by CT using the response evaluation criteria in solid tumors(RECIST 1.1) criteria.Tumor area was assessed by DCUS as enhanced appearance of gastric carcinoma due to tumor vascularity during the contrast phase as compared to the normal gastric wall.Histopathologic analysis was carried out according to the Mandard tumor regression grade criteria and used as the reference standard.Receiver operating characteristic(ROC) analysis was used to evaluate the efficacy of DCUS parameters in differentiating histopathological responders from non-responders.RESULTS:The study population consisted of 32 men and 11 women,with mean age of 59.7 ± 11.4 years.Neither age,sex,histologic type,tumor site,T stage,nor N stage was associated with pathological response.The responders had significantly smaller mean tumor size than the non-responders(15.7 ± 7.4 cm vs 33.3 ± 14.1 cm,P < 0.01).According to Mandard's criteria,27 patients were classified as responders,with 11(40.7%) showing decreased tumor size by DCUS.In contrast,only three(18.8%) of the 16 non-responders showed decreased tumor size by DCUS(P < 0.01).The area under the ROC curve was 0.64,with a 95%CI of 0.46-0.81.The effects of several cut-off points on diagnostic parameters were calculated in the ROC curve analysis.By maximizing Youden's index(sensitivity + specificity-1),the best cut-off point for distinguishing responders from non-responders was determined,which had optimal sensitivity of 62.9% and specificity of 56.3%.Using this cut-off point,the positive and negative predictive values of DCUS for distinguishing responders from non-responders were 70.8% and 47.4%,respectively.The overall accuracy of DCUS for therapeutic response assessment was 60.5%,slightly higher than the 53.5% for CT response assessment with RECIST criteria(P = 0.663).Although the advantage was not statistically significant,likely due to the small number of cases assessed.DCUS was able to identify decreased perfusion in responders who showed no morphological change by CT imaging,which can be occluded by such treatment effects as fibrosis and edema.CONCLUSION:DCUS may represent an innovative tool for more accurately predicting histopathological response to neoadjuvant chemotherapy before surgical resection in patients with locally-advanced gastric cancer.Jian Ang Liang Hu Pin-Tong Huang Jin-Xiu Wu Ling-Na Huang Chun-Hui Cao Yi-Xiong Zheng Li Chen 2012World Journal of Gastroenterology2012,18,47:18

3The Clinical Outcomes of Three Surgical Managements on Primary Angle-closure Glaucoma显示文摘Purpose: To investigate the efficacy of trabeculectomy, phacotrabeculectomy and phaco-emulsification in the management of primary angle closure glaucoma (PACG). Methods: A prospective observational study was performed in 88 chronic PACG patients (97 eyes) who were divided into three groups following defined indications to receive different surgical interventions. The indications and clinical outcomes were evaluated. The mean follow-up was (17.7±4.9) months. Results: Success rate in trabeculectomy, phacotrabeculectomy and phacoemulsification group was 81.08%, 78.57‰ and 81.25‰, respectively. The anterior chamber depth was deeper and the angle was wider postoperatively vs. preoperatively both in phaco-trabeculectomy and phacoemulsification group. No obvious changes were seen in trabeculectomy group. The coefficient of outflow facility of aqueous humor (C values) significantly increased in three groups postoperatively (P < 0.01). No severe intra-operative complications were found and the incidence of postoperative complications was low. Five eyes sustained hypotony 3±1.87 months in trabeculectomy group and 1 eye happened malignant glaucoma in phacotrabeculectomy group. The visual acuity in patients with phacotrabeculectomy plus intraocular lens implantation and those only with phacoemulsification plus intraocular lens implantation were improved 78.57% and 93.74%, respectively. No significant improvement was found in trabeculectomy group (χ2 = 47.10, P < 0.001). Conclusion: Three surgical interventions were beneficial to manage PACG and with co-existing cataract. The indication choosing was suggested according to visual acuity, angle closure circumference, cataract, medication requirements and optic nerve damage. 5Output Waveform Analysis of an Electro-hydraulic Vibrator Controlled by the Multiple Valves显示文摘The existing research of the electro-hydraulic vibrator mainly focuses on system stability,working frequency width and output waveform distortion.However,this high frequency performance of the electro-hydraulic vibrator is difficult to be improved greatly due to fast insufficiently frequency response of the servo valve itself and limited compensation capability of the control structure in the vibrator system.In this paper,to realize high frequency vibration,an improved two-dimensional valve(here within defined as a2D valve)as a main control component is adopted to the parallel connection with a servo valve to control the electro-hydraulic vibrator.Because the output waveforms of this electro-hydraulic vibrator are incapable to be verified through timely feedback as in the conventional electro-hydraulic servo system,the analysis to the output waveform becomes crucial to the design and control of the electro-hydraulic vibrator.The mathematical models of hydraulic actuation mechanism and the orifice area of the parallel valves connection are established first.And then the vibration process is divided into two sections in terms of the direction of the flow,the analytical expression of the excited waveform is solved.Based on the analytical results,the vibration boundary positions and the relationships exist between working states and the control parameters are derived.Finally an experimental system was built to validate the theoretical analysis.It is verified that this electro-hydraulic vibration system could achieve high working frequency,up to 2 000 Hz.The excited waveform is similar to the sinusoidal waveform.And the ascent and decent slopes of the waveforms are somewhat asymmetrical.This asymmetry is not only caused by the change of the direction of the elastic force but also dependent on the bias position of the vibration.Consequently the distortion of effective working waveform is less than 10%.This electro-hydraulic vibrator controlled by the multiple valves could not only greatly enhance the working frequency but also precisely control the vibration characteristic variables such as waveform shape.REN Yan RUAN Jian JIA Wen'ang 2014Chinese Journal of Mechanical Engineering2014,27,1:10
6Separate Control of High Frequency Electro-hydraulic Vibration Exciter显示文摘The working frequency of the conventional electro-hydraulic vibration exciters,which consist of a servo valve and a hydraulic cylinder,is generally restricted within a narrow range due to limited frequency response capability of the servo valve itself.To counteract such restriction,a novel scheme for an electro-hydraulic vibrator,controlled by a two-dimensional valve(2D valve) and a bias valve in parallel,is therefore proposed.The frequency,amplitude and offset are independently controlled by rotary speed,axial sliding of the spool of the 2D valve and axial sliding of the spool of the bias valve.The principle of separate control was presented and the regulation approach of frequency,amplitude and offset was discussed.A mathematical model of the hydraulic power mechanism for the proposed vibration exciter was established to investigate the relationship between the amplitude and the axial sliding of the 2D valve' spool,as well as that between the offset and the axial sliding of the bias valve's spool at various frequencies.An experimental system was built to validate the theoretical analysis.It is verified that the 2D exciter is capable of working smoothly in a frequency range of 5- 200 Hz.And its frequency,amplitude and offset can be controlled respectively by either closed loop or open loop method.There is a linear relationship between the output amplitude and the spool axial opening of the 2D valve until a point when the flow rate becomes saturate and the amplitude remains constant.The offset displacement of the cylinder's piston is linearly proportional to the axial displacement of the spool of the bias valve,when the valve opening is less than 25%.Thereafter,the slop of the offset curve decreases and tends to saturate.The proposed electro-hydraulic vibration controlled by the 2D valve not only facilitates the realization of high-frequency electro-hydraulic vibration,the high-accuracy of vibration can also be achieved by means of independent controls to the frequency,amplitude and offset.JIA Wen'ang RUAN Jian REN Yan 2011Chinese Journal of Mechanical Engineering2011,24,2:7

8Phase characterisation of metalenses显示文摘Metalenses have emerged as a new optical element or system in recent years,showing superior performance and abundant applications.However,the phase distribution of a metalens has not been measured directly up to now,hindering further quantitative evaluation of its performance.We have developed an interferometric imaging phase measurement system to measure the phase distribution of a metalens by taking only one photo of the interference pattern.Based on the measured phase distribution,we analyse the negative chromatic aberration effect of monochromatic metalenses and propose a feature size of metalenses.Different sensitivities of the phase response to wavelength between the Pancharatnam-Berry phase-based metalens and propagation phase-reliant metalens are directly observed in the experiment.Furthermore,through phase distribution analysis,it is found that the distance between the measured metalens and the brightest spot of focusing will deviate from the focal length when the metalens has a low nominal numerical aperture,even though the metalens is ideal without any fabrication error.We also use the measured phase distribution to quantitatively characterise the imaging performance of the metalens.Our phase measurement system will help not only designers optimise the designs of metalenses but also fabricants distinguish defects to improve the fabrication process,which will pave the way for metalenses in industrial applications.Maoxiong Zhao Mu Ku Chen Ze-Peng Zhuang Yiwen Zhang Ang Chen Qinmiao Chen Wenzhe Liu Jiajun Wang Ze-Ming Chen Bo Wang Xiaohan Liu Haiwei Yin Shumin Xiao Lei Shi Jian-Wen Dong Jian Zi Din Ping Tsai 2021Light(Science & Applications)2021,10,4:1

12Risk factors for sepsis-associated encephalopathy显示文摘Sepsis-associated encephalopathy(SAE) is a diffuse and acute cerebral dysfunction caused by sepsis.Many sepsis patients exhibit acute deterioration in mental status during the early stage of disease,and central nervous system dysfunction has been shown to increase patient mortality.The present study selected 284 sepsis patients who were admitted to the Intensive Care Unit of Beijing Friendship Hospital,Capital Medical University,from January to December 2009.The patients were assigned to SAE and non-SAE patient groups according to SAE occurrence.SAE incidence was 37.68%,and mortality was significantly greater in SAE patients compared with non-SAE patients(41.12% vs.17.51%,P < 0.01).Univariate analysis and multivariate logistic regression analysis indicated lower arterial partial pressure of oxygen and greater alanine aminotransferase and Acute Physiology and Chronic Health Evaluation II scores in the SAE group compared with the non-SAE group.Arterial partial pressure of oxygen,alanine aminotransferase,and Acute Physiology and Chronic Health Evaluation II scores were determined to be potential risk factors for SAE.Jian Li Ang Li Yibing Weng Shuwen Zhang Meili Duan 2011Neural Regeneration Research2011,6,4:1
13Effect of Meridional Characteristic Angle on Aerodynamic Performance of Ram-Rotor显示文摘A kind of meridional concept which has a shape of contraction-expansion(Con-Exp)was applied to the ram-rotor in the paper,and detailed numerical simulation to understand the impact of meridional shape and geometric characteristic angle on the aerodynamic behavior of ram-rotors has been undertaken.Compared to baseline,there was a positive effect in intensifying shock train and suppressing low momentum fluid in Con-Exp scheme,and the pressure ratio and efficiency of Con-Exp scheme increased by 2.075%and 3.160%,respectively.When the contracting angle was increasing,supersonic flow at and after throat region accelerated remarkably,and the static pressure rise was decreased gradually.The terminating shock train was transformed to terminating shock,and the efficiency of ram-rotor was unfavorable to improve due to low throttle extent.As expanding angle rose,the terminating shock moved forward and turned to shock train gradually.The pressure rise capacity and throttling ability of ram-rotor were raised.More low momentum fluid was accumulated near the middle and upper flow-path,yielding greater flow loss at large expanding angle.HAN Ji'ang GUAN Jian ZHONG Jingjun CHEN Luting 2020Journal of Thermal Science2020,29,3:1

15The race to the nociceptor: mechanical versus temperature effects in thermal pain of dental neurons显示文摘The sensing of hot and cold stimuli by dental neurons differs in several fundamental ways. These sensations have been characterized quantitatively through the measured time course of neural discharge signals that result from hot or cold stimuli applied to the teeth of animal models. Although various hypotheses have been proposed to explain the underlying mechanism, the ability to test competing hypotheses against experimental recorded data using biophysical models has been hindered by limitations in our understanding of the specific ion channels involved in nociception of dental neurons. Here we apply recent advances in established biophysical models to test the competing hypotheses. We show that a sharp shooting pain sensa-tion experienced shortly following cold stimulation cannot be attributed to the activation of thermosensitive ion channels, thereby falsifying the so-called neuronal hypothesis,which states that rapidly transduced sensations of coldness are related to thermosensitive ion channels. Our results support a central role of mechanosensitive ion channels and the associated hydrodynamic hypothesis. In addition to the hydrodynamic hypothesis, we also demonstrate that the long time delay of dental neuron responses after hot stimulation could be attributed to the neuronal hypothesis—that a relatively long time is required for the temperature around nociceptors to reach some threshold. 20Acquisition of a novel conjugative multidrug-resistant hypervirulent plasmid leads to hypervirulence in clinical carbapenem-resistant Klebsiella pneumoniae strains显示文摘The co-occurrence of plasmid-mediated multidrug resistance and hypervirulence in epidemic carbapenem-resistant Klebsiella pneumoniae has emerged as a global public health issue.In this study,an ST23 carbapenem-resistant hyper-virulent K.pneumoniae(CR-HvKP)strain VH1-2 was identified from cucumber in China and harbored a novel hybrid plasmid pVH1-2-VIR.The plasmid pVH1-2-VIR carrying both virulence and multidrug-resistance(MDR)genes was likely generated through the recombination of a virulence plasmid and an IncFIIK conjugative MDR plasmid in clinical ST2318622 isolated from a sputum sample.The plasmid pVH1-2-VIR exhibited the capacity for transfer to the clinical ST11 carbapenem-resistant K.pneumoniae(CRKP)strain via conjugation assay.Acquisition of pVH1-2-VIR plasmid directly converted a CRKP into CR-HvKP strain characterized by hypermucoviscosity,heightened virulence for Galleria mellonella larvae,and increased colonization ability in the mouse intestine.The emergence of such a hybrid plasmid may expedite the spread of CR-HvKP strains,posing a significant risk to human health.Gong Li Ling Jia Lei Wan Lijuan Xia Ang Gao Runshi Yang Ruanyang Sun Minge Wang Juan Du Xinlei Lian Rongmin Zhang Liangxing Fang Xiaoping Liao Yahong Liu Bao-Tao Liu Jian Sun 2023mLife2023,2,3:0
