1Entecavir vs lamivudine therapy for na?ve patients with spontaneous reactivation of hepatitis B presenting as acute-on-chronic liver failure显示文摘AIM:To investigate the short-term and long-term efficacy of entecavir versus lamivudine in patients with spontaneous reactivation of hepatitis B presenting as acute-on-chronic liver failure(ACLF).METHODS:This was a single center,prospective cohort study.Eligible,consecutive hospitalized patients received either entecavir 0.5 mg/d or lamivudine 100mg/d.All patients were given standard comprehensive internal medicine.The primary endpoint was survival rate at day 60,and secondary endpoints were reduction in hepatitis B virus(HBV)DNA and alanine aminotransferase(ALT)levels,and improvement in Child-Turcotte-Pugh(CTP)and model for end-stage liver disease(MELD)scores at day 60 and survival rate at week 52.RESULTS:One hundred and nineteen eligible subjects were recruited from 176 patients with severe acute exacerbation of chronic hepatitis B:65 were included in the entecavir group and 54 in the lamivudine group(full analysis set).No significant differences were found in patient baseline clinical parameters.At day 60,entecavir did not improve the probability of survival(P=0.066),despite resulting in faster virological suppression(P<0.001),higher rates of virological response(P<0.05)and greater reductions in the CTP and MELD scores(all P<0.05)than lamivudine.Intriguingly,at week 52,the probability of survival was higher in the entecavir group than in the lamivudine group[42/65(64.6%)vs 26/54(48.1%),respectively;P=0.038].The pretreatment MELD score(B,1.357;95%Cl:2.138-7.062;P=0.000)and virological response at day30(B,1.556;95%Cl:1.811-12.411;P=0.002),were found to be good predictors for 52-wk survival.CONCLUSION:Entecavir significantly reduced HBV DNA levels,decreased the CTP and MELD scores,and thereby improved the long-term survival rate in patients with spontaneous reactivation of hepatitis B presenting as ACLF.Yang Zhang Xiao-Yu Hu Sen Zhong Fang Yang Tao-You Zhou Guo Chen Yan-Yan Wang Jian-Xing Luo 2014World Journal of Gastroenterology2014,20,16:42
2New vacuum solar telescope and observations with high resolution显示文摘The New Vacuum Solar Telescope(NVST) is a one meter vacuum solar telescope that aims to observe fine structures on the Sun. The main goals of NVST are high resolution imaging and spectral observations, including measurements of the solar magnetic field. NVST is the primary ground-based facility used by the Chinese solar research community in this solar cycle. It is located by Fuxian Lake in southwest China, where the seeing is good enough to perform high resolution observations. We first introduce the general conditions at the Fuxian Solar Observatory and the primary science cases of NVST. Then, the basic structures of this telescope and instruments are described in detail. 3Effect of Sancaijiangtang on plasma nitric oxide and endothelin-1 levels in patients with type 2 diabetes mellitus and vascular dementia:a single-blind randomized controlled trial显示文摘OBJECTIVE: To observe the effect of Sancaijiangtang powders on plasma nitric oxide and endothelin-1 levels. We sought to identify the common pathological link and mechanism of action for Traditional Chinese medicine in type 2 diabetes mellitus and vascular dementia,and to explicate the material basis for treating the different diseases with the same method in Traditional Chinese Medicine.METHODS: In total,168 patients with type 2 diabetes mellitus and vascular dementia were enrolled in the study,and randomly divided into two groups by simple randomization. Patients in the treatment group received oral Sancaijiangtang powders with pioglitazone hydrochloride three times daily,while patients in the control group received pioglitazone hydrochloride alone. The treatment course was for12 weeks. Mini-mental state examinations(Chinese version) and Montreal Cognitive Assessments(Beijing version) were performed,and fasting plasma glucose,fasting insulin,hemoglobin A1 c,homeostasis model assessment of insulin resistance,plasma nitric oxide and endothelin-1 levels were measured before and after the treatment.RESULTS: The post-treatment levels for all measurements in both groups were better than pre-treatment levels(P < 0.05). The post-treatment levels for all measurements in the treatment group were better than the levels measured in the control group(P < 0.05).CONCLUSION: Type 2 diabetes mellitus and vascular dementia have common pathological mechanisms for insulin resistance and endothelium dysfunction. Sancaijiangtang powders could improve the release of nitric oxide and inhibit the secretion of endothelin-1. 4On comparison of modified ADRCs for nonlinear uncertain systems with time delay显示文摘To tackle systems with both uncertainties and time delays, several modified active disturbance rejection control(ADRC) methods, including delayed designed ADRC(DD-ADRC), polynomial based predictive ADRC(PP-ADRC), Smith predictor based ADRC(SP-ADRC) and predictor observer based ADRC(PO-ADRC), have been proposed in the past years. This paper is aimed at rigorously investigating the performance of these modified ADRCs, such that the improvements of each method can be demonstrated.The capability to tackle time delay, the necessity of stable open loop and the performance of rejecting uncertainties for these methods are fully studied and compared. It is proven that large time delay cannot be tolerated for the stability of the closed-loop systems based on DD-ADRC and PP-ADRC. Moreover, stable open loop is shown to be necessary for stabilizing the closed-loop systems based on SP-ADRC. Furthermore,the performance of rejecting the 'total disturbance' at low frequency for these modified ADRCs is evaluated and quantitatively discussed. 7Olivine-norite rock detected by the lunar rover Yutu-2 likely crystallized from the SPA-impact melt pool显示文摘Chang’E-4 landed in the South Pole-Aitken(SPA)basin,providing a unique chance to probe the composition of the lunar interior.Its landing site is located on ejecta strips in Von Karm´an crater that´possibly originate from the neighboring Finsen crater.A surface rock and the lunar regolith at 10 sites along the rover Yutu-2 track were measured by the onboard Visible and Near-Infrared Imaging Spectrometer in the first three lunar days of mission operations.In situ spectra of the regolith have peak band positions at 1 and 2μm,similar to the spectral data of Finsen ejecta from the Moon Mineralogy Mapper,which confirms that the regolith’s composition of the landing area is mostly similar to that of Finsen ejecta.The rock spectrum shows similar band peak positions,but stronger absorptions,suggesting relatively fresh exposure.The rock may consist of 38.1±5.4%low-Ca pyroxene,13.9±5.1%olivine and 48.0±3.1%plagioclase,referred to as olivine-norite.The plagioclase-abundant and olivine-poor modal composition of the rock is inconsistent with the origin of the mantle,but representative of the lunar lower crust.Alternatively,the rock crystallized from the impact-derived melt pool formed by the SPA-impact event via mixing the lunar crust and mantle materials.This scenario is consistent with fast-cooling thermal conditions of a shallow melt pool,indicated by the fine to medium-sized texture(<3 mm)of the rock and the SPA-impact melting model[Icarus 2012;220:730–43].Honglei Lin Zhiping He Wei Yang Yangting Lin Rui Xu Chi Zhang Meng-Hua Zhu Rui Chang Jinhai Zhang Chunlai Li Hongyu Lin Yang Liu Sheng Gou Yong Wei Sen Hu Changbin Xue Jianfeng Yang Jie Zhong Xiaohui Fu Weixing Wan Yongliao Zou 2020National Science Review2020,7,5:9
8Comparison of screening strategies for Lynch syndrome in patients with newly diagnosed endometrial cancer:a prospective cohort study in China显示文摘Background: The prevalence of Lynch syndrome and screening strategies for this disorder in Chinese patients with endometrial cancer have seldom been investigated. Such data would be essential for the screening, prevention, genetic counseling, and treatment of Lynch syndrome. The purpose of this prospective study was to determine the accuracy of the mismatch repair (MMR) protein immunohistochemistry (IHC), microsatellite instability (MSI) test, and clinical diagnostic criteria in screening for Lynch syndrome-associated endometrial cancer (LS-EC) in a prospective Chinese cohort. Methods: All patients with newly diagnosed endometrial cancer (EC) were evaluated using clinical diagnostic criteria (Amsterdam II criteria and the revised Bethesda guidelines), MSI test, and IHC of MMR proteins in tumor tissues. For all patients, the screening results were compared with results of germline sequencing for pathogenic variants of MMR genes. Results: Between December 2017 and August 2018, a total of 111 unselected patients with newly diagnosed EC were enrolled. Six patients (5.4%) harbored a pathogenic germline mutation of MMR genes: 1 had a mutation in MutL homolog 1 (MLH1), 2 in MutS homolog 2 (MSH2), and 3 in MutS homolog 6 (MSH6). The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for identifying LS-EC were 33.3%, 88.6%, 14.3%, and 95.9%, for the clinical criteria, 66.7%, 75.0%, 14.3%, and 97.3% for IHC of MMR proteins, 100%, 89.9%, 33.3%, and 100% for MSI test, and 100%, 72.4%, 20.0% and 100% for combined IHC and MSI test, respectively. The combination of IHC and MSI test had higher sensitivity and PPV than the clinical criteria (p = 0.030). MSI test and IHC were highly concordant for LS-EC screening (73/77, 94.8%). 9Sedimentary and geochemical characteristics of the Triassic Chang 7 Member shale in the Southeastern Ordos Basin,Central China显示文摘The Ordos Basin is the largest petroliferous basin in China, where the Chang 7 Member shale serves as the major source rock in the basin, with an area of more than 100,000 km^2 So far, sedimentary and geochemical characterizations have rarely been conducted on the shale in shallow(< 1000 m) areas in the southeastern part of the basin, but such characterizations can help identify the genesis of organic-rich shale and promote the prediction and recovery of shale oil. In this paper,several outcrop sections of the Chang 7 Member in the Tongchuan area were observed and sampled, and sedimentary and geochemical characterizations were conducted for the well-outcropped YSC section. The study results show that the Chang7 Member shale is widely distributed laterally with variable thickness. The organic-rich shale is 7-25 m thick in total and exhibits obvious horizontal variation in mineral composition. In the eastern sections, the shale contains organic matter of TypeⅡ_2-Ⅲ and is low in thermal maturity, with high clay mineral content, low K-feldspar content, and no pyrite. In the western sections, the shale contains Type Ⅱ_1 organic matter and is low in thermal maturity, with high clay mineral, K-feldspar, and pyrite contents. The YSC section reveals three obvious intervals in vertical mineral composition and organic abundance.The Chang 7 Member organic-rich shale(TOC > 10%) contains mainly sapropelite and liptinite, with Type Ⅱ kerogen. It is generally characterized by a hydrocarbon potential of more than 70 mg/g, low maturity, and shallow-semideep lacustrine facies. In the western sections, the shale, still in a low maturity stage, has a higher hydrocarbon potential and is optional for shale oil recovery. 10Distribution of Traditional Chinese Medicine patterns in 324 cases with hepatitis B-related acute-on-chronic liver failure:a prospective,cross-sectional survey显示文摘OBJECTIVE:To determine the distribution of Traditional Chinese Medicine(TCM) patterns in hepatitis B-related acute-on-chronic liver failure(HB-ACLF) in different stages for guiding clinical prescriptions and treatments.METHODS:A prospective,cross-sectional survey method was used in this study.A total of 324 cases with HB-ACLF in China were involved.RESULTS:The general frequency of TCM patterns in HB-ACLF were as follows:Heat Toxin Stagnation Pattern(134/324,41.36%),Damp-heat Obstruction Pattern(66/324,20.37%),Yang Qi Deficiency Pattern(52/324,16.05%),and Liver and Kidney Yin Deficiency Pattern(26/324,8.02%).In the early stage of HB-ACLF,there was a remarkably higher percentage of excessive patterns than those in the middle and late stage.The incidence of Heat Toxin Stagnation reached 58.57%(82/140) in the early stage,while it was 33.96%(36/106) in the middle stage and 20.51%(16/78) in the late stage.In the early stage of HB-ACLF,excessive patterns,such as the Heat Toxin Stagnation Pattern,were more prevalent than those in the middle and late stages(P<α ' = 0.003).However,in the late stage of HB-ACLF,deficient patterns,such as the Yang Qi Deficiency Pattern,were more prevalent than those in the early and middle stages.The Yang Qi Deficiency Pattern had a higher rate of 41.03%(32/78) in the late stage compared with that of 20.75%(22/106) in the middle stage and 8.57%(12/140,P<α ' =0.003) in the early stage.The distribution of the other patterns was not significant between the three stages(P>0.003).CONCLUSIONS:There are four major patterns of HB-ACLF,including the Heat Toxin Stagnation Pattern,the Damp-heat Obstruction Pattern,the Yang Qi Deficiency Pattern,and the Liver and Kidney Yin Deficiency Pattern.The Heat Toxin Stagnation and Yang Qi Deficiency Patterns are the representative patterns in the early and late stages of HB-ACLF.In the middle stage of HB-ACLF,the TCM patterns vary in a complicated manner,with no significant difference among the patterns.Treatment for HB-ACLF should vary with the different representative patterns in the early and late stages.Xiaoyu Hu Yang Zhang Guo Chen Yibei Li Sen Zhong 2012Journal of Traditional Chinese Medicine2012,32,4:6

13Role of LAP^+CD4^+ T cells in the tumor microenvironment of colorectal cancer显示文摘AIM To investigate the abundance and potential functions of LAP^+CD4^+ T cells in colorectal cancer(CRC). METHODS Proportions of LAP^+CD4^+ T cells were examined in peripheral blood and tumor/paratumor tissues of CRC patients and healthy controls using flow cytometry. Expression of phenotypic markers such as forkhead box(Fox)p3, cytotoxic T-lymphocyte-associated protein(CTLA)-4, chemokine CC receptor (CCR)4 and CCR5 was measured using flow cytometry. LAP^-CD4^+ and LAP^+CD4^+ T cells were isolated using a magnetic cellsorting system and cell purity was analyzed by flow cytometry. Real-time quantitative polymerase chain reaction was used to measure expression of cytokines interleukin (IL)-10 and transforming growth factor(TGF)-β.RESULTS The proportion of LAP^+CD4^+ T cells was significantly higher in peripheral blood from patients (9.44% ± 3.18%) than healthy controls (1.49% ± 1.00%, P < 0.001). Among patients, the proportion of LAP^+CD4^+ T cells was significantly higher in tumor tissues(11.76% ± 3.74%) compared with paratumor tissues (3.87% ± 1.64%, P < 0.001). We also observed positive correlations between the proportion of LAP^+CD4^+ T cells and TNM stage(P < 0.001), distant metastasis(P < 0.001) and serum level of carcinoembryonic antigen(P < 0.05). Magnetic-activated cell sorting gave an overall enrichment of LAP^+CD4^+ T cells (95.02% ± 2.87%), which was similar for LAP^-CD4^+ T cells(94.75% ± 2.76%). In contrast to LAP^-CD4^+ T cells, LAP^+CD4^+ T cells showed lower Foxp3 expression but significantly higher levels of CTLA-4, CCR4 and CCR5(P < 0.01). 17Enhanced performances of polypropylene membranes by molecular layer deposition of polyimide显示文摘Molecular layer deposition(MLD) for the deposition of polyimide(PI) at low temperature of 110 °C has been firstly introduced into the field of membrane separation. With the optimized MLD deposition parameters, such low deposition temperature has successfully expanded the application of MLD for the surface modification of polymeric materials. Globular PI particulates grow on both the free surfaces as well as the pore walls of the polypropylene(PP) membranes as isolated islands during progressive precursor exposures. The PI-deposited PP membranes exhibit synergistically improved performances in various aspects. Evidently improved surface hydrophilicity and permeation performance(30%) have been achieved via the MLD deposition of polyimide films. The overall separation efficiency maintained higher than 85% even after 250 cycles of MLD deposition.
