| 2 | De novo combined lamivudine and adefovir dipivoxil therapy vs entecavir monotherapy for hepatitis B virus-related decompensated cirrhosis显示文摘AIM:To compare efficacy of combined lamivudine(LAM)and adefovir dipivoxil(ADV)therapy with that of entecavir(ETV)monotherapy for hepatitis B virus(HBV)-related decompensated liver cirrhosis.METHODS:A total of 120 na ve patients with HBVrelated decompensated cirrhosis participated in this study.Sixty patients were treated with combined LAM and ADV therapy(LAM+ADV group),while the other60 were treated with ETV monotherapy(ETV group)for two years.Tests for liver and kidney function,alpha-fetoprotein,HBV serum markers,HBV DNA load,prothrombin time(PT),and ultrasonography or computed tomography scan of the liver were performed every1 to 3 mo.Repeated measure ANOVA and theχ2test were performed to compare the efficacy,side effects,and the cumulative survival rates at 48 and 96 wk.RESULTS:Forty-five patients in each group were observed for 96 wk.No significant differences in HBV DNA negative rates and alanine aminotransferase(ALT)normalization rates at weeks 48(χ2=2.12 and 2.88)and96(χ2=3.21 and 3.24)between the two groups were observed.Hepatitis B e antigen seroconversion rate in the LAM+ADV group at week 96 was significantly higher in the ETV group(43.5%vs 36.4%,χ2=4.09,P<0.05).Viral breakthrough occurred in 2 cases(4.4%)by week 48 and in 3 cases(6.7%)by week 96 in the LAM+ADV group,and no viral mutation was detected.In the ETV group,viral breakthrough occurred in 1 case(2.2%)at the end of week 96.An increase in albumin(F=18.9 and 17.3),decrease in total bilirubin and in ALT(F=16.5,17.1 and 23.7,24.8),reduced PT(F=22.7 and 24.5),and improved Child-Turcotte-Pugh and the model for end-stage liver disease scores(F=18.5,17.8,and 24.2,23.8)were observed in both groups.The cumulative rates of mortality and liver transplantation were 16.7%(10/60)and 18.3%(11/60)in the LAM+ADV and ETV groups,respectively.CONCLUSION:Both LAM+ADV combination therapy and ETV monotherapy can effectively inhibit HBV replication,improve liver function,and decrease mortality. | Jiang-Shan Lian Lin-Yan Zeng Jian-Yang Chen Hong-Yu Jia Yi-Min Zhang Dai-Rong Xiang Liang Yu Jian-Hua Hu Ying-Feng Lu Ling Zheng Lan-Juan Li Yi-Da Yang | 2013 | World Journal of Gastroenterology2013,19,37: | 36 |

It is very important to increase FUE in China for its relatively low FUE and serious losses of nutrients. Recent advances of the technologies to increase FUE are reviewed in this article. These include site-specific and real-time nitrogen management, non-destructive quick test of the nitrogen status of plants, new types of slow release and controlled release fertilizers, site-specific nutrient management, and use of urease inhibitor and nitrification inhibitor to decrease nitrogen losses. | 5 | Appropriate treatment strategies improve survival of hepatocellular carcinoma patients with portal vein tumor thrombus显示文摘AIM:To evaluate the survival benefits of different treatment strategies for hepatocellular carcinoma(HCC)patients with portal vein tumor thrombus(PVTT)and to determine the prognosis factors.METHODS:Between 2007 and 2009,338 HCC patients treated for PVTT were retrospectively studied.The patients were divided into 4 groups that underwent different treatments:the conservative treatment group(n=75),the transarterial chemoembolization(TACE)group(n=86),the hepatic resection group(n=90),and the hepatic resection associated with postoperative TACE group(n=87).Survival rates were determined using the Kaplan-Meier method and differences between the groups were identified through log-rankanalysis.Cox’s proportional hazard model was used to identify the risk factors for survival.RESULTS:The mean survival periods for patients in the conservative treatment,TACE,hepatic resection and hepatic resection associated with postoperative TACE groups were 3.8,7,8.2 and 15.1 mo,respectively.Significant differences were observed in the survival rates.For the surgical resection associated with postoperative TACE group,the survival rates after 1,2 and3 years were 49%,37%and 19%,respectively.These results were significantly higher than those of the other groups(P<0.05).Meanwhile,the 1,2 and 3 year survival rates for the surgical resection group were 28%,20%and 15%,whereas those for the TACE group were17.5%,0%and 0%,respectively.These values significantly increased after hepatic resection compared with those after TACE(P<0.05).CONCLUSION:Surgical resection is the most effective therapeutic strategy for HCC patients with PVTT and results in high hepatic functional reserve.For patients who can tolerate the procedure,postoperative TACE is necessary to prevent recurrence and prolong the survival period. | Jia-Zhou Ye Yong-Quan Zhang Hai-Hong Ye Tao Bai Liang Ma Bang-De Xiang Le-Qun Li | 2014 | World Journal of Gastroenterology2014,20,45: | 32 |

| 8 | Early kidney injury during long-term adefovir dipivoxil therapy for chronic hepatitis B显示文摘AIM: To evaluate urine β2-microglobulin(β2-M), retinol-binding protein(RBP) excretion, and renal impairment with adefovir dipivoxil(ADV) for chronic hepatitis B. METHODS: We enrolled 165 patients with chronic hepatitis B infection who were treated with ADV monotherapy(n = 90) or ADV plus lamivudine combination therapy(n = 75). An additional 165 chronic hepatitis B patients treated with entecavir were recruited as controls. We detected serum creatinine, urine β2-M, and RBP levels, and estimated the glomerular filtration rate(e GFR) at the initiation of antiviral therapy and every 6 mo for a period of five years. RESULTS: Urine β2-M abnormalities were observed in patients during the first(n = 3), second(n = 7), third(n = 11), fourth(n = 16), and fifth(n = 21) year of ADV treatment. Urinary RBP abnormalities were observed in patients during the first(n = 2), second(n = 8), third(n = 12), fourth(n = 15), and fifth(n = 22) year of ADV treatment. e GFR decreased 20%-30% from baseline in 20 patients, 30%-50% in 12 patients, and > 50% in 3 patients during the five years of treatment. Further analysis indicated that decreases in e GFR of ≥ 30% relative to the baseline level correlated significantly with urine RBP and β2-M abnormalities. In contrast, both serum creatinine and e GFR remained stable in patients treated with entecavir, and only one of these patients developed a urine β2-M abnormality, and two developed urine RBP abnormalities during the five years of treatment. | 10 | Application of nonlinear chemical fingerprinting to identification,evaluation and clinical use of Glycyrrhiza显示文摘Nonlinear chemical fingerprinting is a new technique for analyzing traditional Chinese medicines (TCMs). We developed such a method and applied it to Glycyrrhiza, a significant TCM. Nonlinear chemical fingerprints were obtained for Glycyrrhiza from four different production areas, and for 14 other TCMs with similar induction time. The calculation of their similarities was based on similarity system theory. Using intuitive visual comparison of the data and the similarity calculation results, Glycyrrhiza was discriminated from the 14 other TCMs. There was a linear relationship between the content of active TCM components and the induction time. This was used to compare the content of active components in Glycyrrhiza from the four different production areas, and establish an equivalent-dose relationship for these four samples. | 12 | Dihydromyricetin-mediated inhibition of the Notch1 pathway induces apoptosis in QGY7701 and Hep G2 hepatoma cells显示文摘AIM To investigate whether Dihydromyricetin(DHM) inhibits cell proliferation and promotes apoptosis by downregulating Notch1 expression.METHODS The correlation between Notch1 and Hes1(a Notch1 target molecule) expression in hepatoma samples was confirmed by q RT-PCR. In addition, MTT assays, flow cytometry and TUNEL analysis showed that DHM possessed strong anti-tumor properties, evidenced not only by reduced cell proliferation but also by enhanced apoptosis in QGY7701 and Hep G2 hepatocellular carcinoma(HCC) cells. The expressions of Notch1, Hes1, Bcl-2 and Bax were determined by Western blot.RESULTS Among the tested samples(n = 64), the expression levels of Notch1(75% of patients) and Hes1(79.7% of patients) m RNA in tumor tissues were higher than in the normal liver tissues. There was a negative correlation between the expression of Notch1 and the degree of differentiation and positively correlated with the Alpha Fetal Protein concentration. The viability of HCC cells treated with DHM was significantly inhibited in a dose and time-dependent manner. Apoptosis was induced in Hep G2 and QGY7701 cell lines following 24 h of DHM treatment. After treatment with DHM, the protein expression of Notch1 was downregulated, the apoptosis-related protein Bax was upregulated and Bcl2 was downregulated. Notch1 si RNA further enhanced the anti-tumor properties of DHM. | 13 | Hepatitis B virus and hepatocellular carcinoma at the miRNA level显示文摘AIM: To study Hepatitis B virus (HBV) infection and its association with hepatocellular carcinoma (HCC) at the miRNA level.METHODS: Three cellular models were used to investigate miRNA expression changes during HBV infection: human HepG2 hepatoblastoma cell line as a model without HBV infection;HepG2 cell line transfected with a 1.3-fold full-length HBV genome as an acute infection model;and HepG2.2.15 cell line,which is derived from HepG2 and stably transfected with a complete HBV genome,as a chronic infection model.The miRNA levels were examined using microarray technology.To explore the relationship between HBV infection and HCC genesis at the miRNA level,we downloaded from national center for biotechnology information Gene Expression Omnibus an miRNA expression dataset derived from HCC patients,most of whom are HBV carriers.We compared the miRNA expression alterations during HBV infection with those in HCC patients,by analyzing miRNA expression change profiles statistically.RESULTS: Seventy-seven and 48 miRNAs were differentially expressed during acute and chronic HBV infection,respectively.Among these miRNAs,25 were in common,the intersection of which was significant under the hypergeometric test (P = 1.3 × 10-11).Fourteen miRNAs were observed to change coherently in the acute and chronic infections,with one upregulated and 13 downregulated.Eleven showed inverse changes during the two phases of infection;downregulated in the acute infection and upregulated in the chronic infection.The results imply that common and specific mechanisms exist at the miRNA level during acute and chronic HBV infection.Besides,comparative analysis of the miRNA expression changes during HBV infection with those in HCC indicates that,although miRNA expression changes during HBV infection are distinct from those in HCC patients (P < 2.2 × 10-16),they exhibited significant correlations (P = 0.0229 for acute infection;P = 0.0084 for chronic infection).Perturbation of miRNA expression during chronic HBV infection was closer to that in HCC patients than that during acute HBV infection.This observation implies the contribution of miRNAs to HCC genesis from HBV infection.According to their patterns of differential expression in acute and chronic HBV infection,as well as in HCC,miRNAs of potential research interest could be identified,such as miR-18a/miR-18b,miR-106a,miR-221 and miR-101.For instance,the gradient expression alteration of miR-221 in the above three phases,which is downregulated in acute HBV infection,normally expressed in chronic HBV infection,and upregulated in HCC,indicates that it may be a key effector for progression of the disease.CONCLUSION: Our analysis provides insights into HBV infection and related HCC in relation to miRNAs,and reveals some candidate miRNAs for future studies. | Zhen-Zhen Zhang Xiang Liu De-Qiang Wang Mai-Kun Teng Li-Wen Niu Ai-Long Huang Zhi Liang | 2011 | World Journal of Gastroenterology2011,17,28: | 17 |

METHODS:Rats were divided into SAP group and sham-operation group as a control group(n=6). Rhein,chrysophanol,rheochrysidin,magnolol,hesperidin and naringin in DCQD were quantified in rat serum by high performance liquid chromatography tandem mass spectrometry for studying their pharmacokinetics. RESULTS:Early absorption of each DCQD component was tended to degrade in SAP group after treatment with DCQD by gavage.The Cmax(chrysophanol,P= 0.0059;rheochrysidin,P=0.0288;magnolol,P= 0.0487;hesperidin,P=0.0277;naringin,P=0.0023) and AUC(rhein,P=0.0186;chrysophanol,P=0.0013; magnolol,P=0.001;hesperidin,P=0.0081;naringin, P=0.0272)of DCQD component were obviously lower in SAP group than in control group.The T1/2α of chrysophanol and rheochrysidin(P=0.0467 and 0.0005,respectively)and Tmax of chrysophanol and rheochrysidin(P=0.0101 and 0.0037,respectively) lasted longer in SAP group than in control group. | 20 | Molecular simulation of the CH_4/CO_2/ H_2O adsorption onto the molecular structure of coal显示文摘Clarification of the molecular mechanism underlying the interaction of coal with CH4, CO2, and H2O molecules is the basis for an in-depth understanding of the states of fluid in coal and fluid-induced coal swelling/contraction. In terms of instrumental analysis, molecular simulation technology based on molecular mechanics/dynamics and quantum chemistry is a powerful tool for revealing the relationship between the structure and properties of a substance and understanding the interaction mechanisms of physical-chemical systems. In this study, the giant canonical ensemble Monte Carlo(GCMC) and molecular dynamics(MD) methods were applied to investigate the adsorption behavior of a Yanzhou coal model(C222H185N3O17S5). We explored the adsorption amounts of CH4, CO2, and H2O onto Yanzhou coal, the adsorption conformation, and the impact of oxygen-containing functional groups. Furthermore, we revealed the different adsorption mechanisms of the three substances using isosteric heat of adsorption and energy change data.(1) The adsorption isotherms of the mono-component CH4, CO2, and H2O were consistent with the Langmuir model, and their adsorption amounts showed an order of CH4CO2>CH4. In addition, at higher temperatures, the isosteric heat of adsorption decreased; pressure had no significant effect on the heat of adsorption.(3) CH4 molecules displayed an aggregated distribution in the pores, whereas CO2 molecules were cross arranged in pairs. Regarding H2O molecules, under the influence of hydrogen bonds, the O atom pointed to surrounding H2O molecules or the H atoms of coal molecules in a regular pattern. The intermolecular distances of the three substances were 0.421, 0.553, and 0.290 nm, respectively. The radial distribution function(RDF) analysis showed that H2O molecules were arranged in the most compact fashion, forming a tight molecular layer.(4) H2O molecules showed a significantly stratified distribution around oxygen-containing functional groups on the coal surface, and the bonding strength showed a descending order of hydroxyl> carboxyl>carbonyl. In contrast, CO2 and CH4 showed only slightly stratified distributions.(5) After the adsorption of CH4, CO2, and H2O, the total energy, the energy of valence electrons, and the non-bonding interaction of the system in the Yanzhou coal model all decreased. The results regarding the decrease in the total energy of the system indicated an order of H2O>CO2>CH4 in terms of the adsorption priority of the Yanzhou coal model. The results regarding the decrease in the energy of valence electrons showed that under certain geological conditions, a pressure-induced 'coal strain' could lead to a structural rearrangement during the interaction of coal with fluid to form a more stable conformation, which might be the molecular mechanism of coal swelling resulting from the interaction between fluid and coal. An analysis of the contribution of Van der Waals forces, electrostatic interactions and hydrogen bonds to the decrease in non-bonding interactions revealed the mechanism underlying the interactions between coal molecules and the three substances. The interaction between coal molecules and CH4 consisted of typical physical adsorption, whereas that between coal molecules and CO2 consisted mainly of physical adsorption combined with weak chemical adsorption.
