12018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 2Transplantation of collagen scaffold with autologous bone marrow mononuclear cells promotes functional endometrium reconstruction via downregulating ΔNp63 expression in Asherman's syndrome显示文摘Asherman's syndrome(AS) is a common disease that presents endometrial regeneration disorder. However, little is known about its molecular features of this aregenerative endometrium in AS and how to reconstruct the functioning endometrium for the patients with AS. Here, we report that ΔNp63 is significantly upregulated in residual epithelial cells of the impaired endometrium in AS; the upregulated-ΔNp63 induces endometrial quiescence and alteration of stemness. Importantly, we demonstrate that engrafting high density of autologous bone marrow mononuclear cells(BMNCs) loaded in collagen scaffold onto the uterine lining of patients with AS downregulates ΔNp63 expression, reverses ΔNp63-induced pathological changes, normalizes the stemness alterations and restores endometrial regeneration. Finally, five patients achieved successful pregnancies and live births. Therefore, we conclude that ΔNp63 is a crucial therapeutic target for AS. 3The FGOALS climate system model as a modeling tool for supporting climate sciences:An overview显示文摘Climate system models are useful tools for understanding the interactions among the components of the climate system and predicting/projecting future climate change. The development of climate models has been a central focus of the State Key Laboratory of Numerical Modeling for Atmospheric Sciences and Geophysical Fluid Dynamics, Institute of Atmospheric Physics, Chinese Academy of Sciences(LASG/IAP) since the establishment of the laboratory in 1985. In China, many pioneering component models and fully coupled models of the climate system have been developed by LASG/IAP. The fully coupled climate system developed in the recent decade is named FGOALS(Flexible Global Ocean-Atmosphere-Land System Model). In this paper, an application-oriented review of the LASG/IAP FGOALS model is presented. The improved model performances are demonstrated in the context of cloud-radiation processes, Asian monsoon, ENSO phenomena, Atlantic Meridional Overturning Circulation(AMOC) and sea ice. The FGOALS model has contributed to both CMIP5(Coupled Model Intercomparison Project-phase 5) and IPCC(Intergovernmental Panel on Climate Change) AR5(the Fifth Assessment Report). The release of FGOALS data has supported the publication of nearly 500 papers around the world. The results of FGOALS are cited ~106 times in the IPCC WG1(Working Group 1) AR5. In addition to the traditional long-term simulations and projections, near-term decadal climate prediction is a new set of CMIP experiment, progress of LAGS/IAP in the development of nearterm decadal prediction system is reviewed. The FGOALS model has supported many Chinese national-level research projects and contributed to the national climate change assessment report. 4Study on the Driving Style Adaptive Vehicle Longitudinal Control Strategy显示文摘This paper presents a fusion control strategy of adaptive cruise control(ACC) and collision avoidance(CA),which takes into account a driver’s behavioral style. First, a questionnaire survey was performed to identify driver type, and the corresponding driving behavioral data were collected via driving simulator experiments, which served as the template data for the online identification of driver type. Then, the driveradaptive ACC/CA fusion control strategy was designed, and its effect was verified by virtual experiments. 6Acupuncture stimulation of Yamen(GV 15), Fengfu(GV 16), Baihui(GV 20), Shuigou(GV 26) and Hegu(LI 4) reduces brain microglia activation in a traumatic brain injury rat model显示文摘OBJECTIVE: To evaluate the effect of acupuncture on neuroinflammation in traumatic brain injury(TBI) rats by stimulating Yamen(GV 15), Fengfu(GV16), Baihui(GV 20), Shuigou(GV 26) and Hegu(LI 4)acupoints and to investigate the mechanism underpinning this effect.METHODS: A TBI model was induced in SpragueDawley rats using Feeney's freefall impact method.Acupuncture to stimulate the Yamen(GV 15), Fengfu(GV 16), Baihui(GV 20), Shuigou(GV 26) and Hegu(LI 4) acupoints was performed on the TBI rats. After 3 consecutive days of acupuncture treatment, we investigated signal molecules, receptors and microglia related to neuroinflammation in brain tissue of the TBI rats and analyzed the possi-ble mechanism underlying the effect of acupuncture on neuroinflammation.RESULTS: After the acupuncture treatment, ionized calcium binding adaptor molecule 1(Iba1), a protein specific to microglia, was investigated. In the cortical layer of damaged brain tissue in TBI rats,the Iba1-positive area was 3.3% ± 0.9% in the rats that received acupuncture compared with 5.2% ±1.4% in the TBI rats that did not receive acupuncture, and the microglia were smaller with more slender protrusions in the acupuncture-treated rats.This result indicates that acupuncture can significantly reduce microglia activation in TBI rats. A possible mechanism for this effect is that acupuncture reduces the expression of autotaxin and lysophosphatidic acid. Together, these constitute the autotaxin-lysophosphatidic acid axis, which induces microglial activation in the brains of TBI rats. Acupuncture treatment may downregulate the expression of Lysophosphatidic acid(LPA) receptor(LPAR) 1 and LPAR2 on the microglial cytomembrane, which affects the microglia activation process.CONCLUSION: Acupuncture stimulating the Yamen(GV 15), Fengfu(GV 16), Baihui(GV 20),Shuigou(GV 26) and Hegu(LI 4) acupoints can effectively inhibit the development of neuroinflammation after TBI. 7Standard pancreatoduodenectomy versus extended pancreatoduodenectomy with modified retroperitoneal nerve resection in patients with pancreatic head cancer:a multicenter randomized controlled trial显示文摘Background:The extent of pancreatoduodenectomy for pancreatic head cancer remains controversial,and more high-level clinical evidence is needed.This study aimed to evaluate the outcome of extended pancreatoduodenectomy(EPD)with retroperitoneal nerve resection in pancreatic head cancer.Methods:This multicenter randomized trial was performed at 6 Chinese highvolume hospitals that enrolled patients between October 3,2012,and September 21,2017.Four hundred patients with stage I or II pancreatic head cancer and without specific pancreatic cancer treatments(preoperative chemotherapy or chemoradiation)within three months were randomly assigned to undergo standard pancreatoduodenectomy(SPD)or EPD,with the latter followed by dissection of additional lymph nodes(LNs),nerves and soft tissues 270◦on the right side surrounding the superior mesenteric artery and celiac axis.The primary endpoint was overall survival(OS)by intention-to-treat(ITT).The secondary endpoints were disease-free survival(DFS),mortality,morbidity,and postoperative pain intensity.Results:TheR1 ratewas slightly lower with EPD(8.46%)thanwith SPD(12.56%).The morbidity and mortality rates were similar between the two groups.The median OS was similar in the EPD and SPD groups by ITT in the whole study cohort(23.0 vs.20.2 months,P=0.100),while the median DFS was superior in the EPD group(16.1 vs.13.2 months,P=0.031).Patients with preoperative CA19–9<200.0 U/mL had significantly improved OS and DFS with EPD(EPD vs.SPD,30.8 vs.20.9 months,P=0.009;23.4 vs.13.5 months,P<0.001).The EPD group exhibited significantly lower locoregional(16.48%vs.35.20%,P<0.001)andmesenteric LNrecurrence rates(3.98%vs.10.06%,P=0.022).The EPD group exhibited less back pain 6 months postoperation than the SPD group.Conclusions:EPD for pancreatic head cancer did not significantly improve OS,but patients with EPD treatment had significantly improved DFS.In the subgroup analysis,improvements in bothOS and DFS in the EPD armwere observed in patients with preoperative CA19–9<200.0 U/mL.EPD could be used as an effective surgical procedure for patients with pancreatic head cancer,especially those with preoperative CA19–9<200.0 U/mL.Qing Lin Shangyou Zheng Xianjun Yu Meifu Chen Yu Zhou Quanbo Zhou Chonghui Hu Jing Gu Zhongdong Xu LinWang Yimin Liu Qingyu Liu MinWang Guolin Li He Cheng Dongkai Zhou Guodong Liu Zhiqiang Fu Yu Long Yixiong Li Weilin Wang Renyi Qin Zhihua Li Rufu Chen 2023Cancer Communications2023,43,2:3

9Integrating a flipped classroom and problem-based learning into ophthalmology education显示文摘Background: Ophthalmology is an important medical science subject, but it is given with insufficient attention in undergraduate medical education. Flipped classroom(FC) and problem-based learning(PBL) are well-known education methods that can be integrated into ophthalmology education to improve students' competence level and promote active learning. Methods: We used a mixed teaching methodology that integrated a FC and PBL into a 1-week ophthalmology clerkship for 72 fourth-year medical students. The course includes two major sessions: FC session and PBL session, relying on clinical and real-patient cases. Written examinations were set up to assess students' academic performance and questionnaires were designed to evaluate their perceptions. Results: The post-course examination results were higher than the pre-course results, and many students gained ophthalmic knowledge and learning skills to varying levels. Comparison of pre-and post-course questionnaires indicated that interests in ophthalmology increased and more students expressed desires to be eye doctors. Most students were satisfied with the new method, while some suggested the process should be slower and the communication with their teacher needed to strengthen.Conclusions: FC and PBL are complementary methodologies. 10Isolation and Sequence Analysis of NSP2, ORF3 and ORF5 of PRRSV XH-GD Strain from Guangdong Province显示文摘The diseased samples collected from Guangdong Province were inoculated to Marc-145 cells,PRRSV-specific cytopathogenic effects( CPE) such as cell aggregation,cell shrinkage and cell exfoliation were observed and a PRRSV strain XH-GD was obtained. By using primers designed in accordance with the published gene sequences of PRRSV in GenBank,NSP2,ORF3 and ORF5 fragments were amplified by reverse transcription polymerase chain reaction and analyzed by sequence alignment. The results demonstrated that PRRSV strain XH-GD belonged to the North American genotype and the NSP2 gene contained discontinuous deletions of 30 amino acids. NSP2,ORF3 and ORF5 genes of XH-GD shared 83. 3%- 98. 9%,88. 6%- 99. 2% and 88. 1%- 99. 2% nucleotide homology,and76. 8%- 98. 3%,83. 7%- 98. 8% and 88. 1%- 99. 2% amino acid homology with HUB1,NX06,BJsy06,VR-2332,HB-1( sh) 2002,HB-2( sh) 2002,CH-la and RespPRRS MLV,respectively; however,NSP2,ORF3 and ORF5 genes of XH-GD shared only 52. 9%,65. 0% and 64. 3% nucleotide homology,and31. 3%,57. 5% and 58. 9% amino acid homology with LV,respectively. 11Microstructure Evolutions and Properties of Al-Cu Alloy Joint in the Pulsed Power Ultrasonic-Assisted GMAW显示文摘The pulsed power ultrasonic-assisted gas metal arc welding(PU-GMAW)is a new development hybrid welding method.The influences of the pulsed power ultrasonic on the microstructure evolution of the welded joint of Al-Cu alloy were investigated by using scanning electron microscopy,transmission electron microscope and electron back scattering diffraction.The results showed that the efficient heat input in the PU-GMAW was increased by above 100%compared with the traditional GMAW.The grain and eutectic of the PU-GMAW weld seam were refined compared with that of the GMAW.Cavitation and acoustic streaming induced the dendrite fragmentation(α-Al)and heterogeneous nucleation,which were the main reasons for the grain refinement.The microhardness of the PU-GMAW welded joint was improved compared with that of the GMAW owing to the change of the eutectic and grain.Chao Chen Chenglei Fan Zeng Liu Xiaoyu Cai Sanbao Lin Yimin Zhuo 2020Acta Metallurgica Sinica(English Letters)2020,33,10:1
