1De novo combined lamivudine and adefovir dipivoxil therapy vs entecavir monotherapy for hepatitis B virus-related decompensated cirrhosis显示文摘AIM:To compare efficacy of combined lamivudine(LAM)and adefovir dipivoxil(ADV)therapy with that of entecavir(ETV)monotherapy for hepatitis B virus(HBV)-related decompensated liver cirrhosis.METHODS:A total of 120 na ve patients with HBVrelated decompensated cirrhosis participated in this study.Sixty patients were treated with combined LAM and ADV therapy(LAM+ADV group),while the other60 were treated with ETV monotherapy(ETV group)for two years.Tests for liver and kidney function,alpha-fetoprotein,HBV serum markers,HBV DNA load,prothrombin time(PT),and ultrasonography or computed tomography scan of the liver were performed every1 to 3 mo.Repeated measure ANOVA and theχ2test were performed to compare the efficacy,side effects,and the cumulative survival rates at 48 and 96 wk.RESULTS:Forty-five patients in each group were observed for 96 wk.No significant differences in HBV DNA negative rates and alanine aminotransferase(ALT)normalization rates at weeks 48(χ2=2.12 and 2.88)and96(χ2=3.21 and 3.24)between the two groups were observed.Hepatitis B e antigen seroconversion rate in the LAM+ADV group at week 96 was significantly higher in the ETV group(43.5%vs 36.4%,χ2=4.09,P<0.05).Viral breakthrough occurred in 2 cases(4.4%)by week 48 and in 3 cases(6.7%)by week 96 in the LAM+ADV group,and no viral mutation was detected.In the ETV group,viral breakthrough occurred in 1 case(2.2%)at the end of week 96.An increase in albumin(F=18.9 and 17.3),decrease in total bilirubin and in ALT(F=16.5,17.1 and 23.7,24.8),reduced PT(F=22.7 and 24.5),and improved Child-Turcotte-Pugh and the model for end-stage liver disease scores(F=18.5,17.8,and 24.2,23.8)were observed in both groups.The cumulative rates of mortality and liver transplantation were 16.7%(10/60)and 18.3%(11/60)in the LAM+ADV and ETV groups,respectively.CONCLUSION:Both LAM+ADV combination therapy and ETV monotherapy can effectively inhibit HBV replication,improve liver function,and decrease mortality.Jiang-Shan Lian Lin-Yan Zeng Jian-Yang Chen Hong-Yu Jia Yi-Min Zhang Dai-Rong Xiang Liang Yu Jian-Hua Hu Ying-Feng Lu Ling Zheng Lan-Juan Li Yi-Da Yang 2013World Journal of Gastroenterology2013,19,37:36
2Early kidney injury during long-term adefovir dipivoxil therapy for chronic hepatitis B显示文摘AIM: To evaluate urine β2-microglobulin(β2-M), retinol-binding protein(RBP) excretion, and renal impairment with adefovir dipivoxil(ADV) for chronic hepatitis B. METHODS: We enrolled 165 patients with chronic hepatitis B infection who were treated with ADV monotherapy(n = 90) or ADV plus lamivudine combination therapy(n = 75). An additional 165 chronic hepatitis B patients treated with entecavir were recruited as controls. We detected serum creatinine, urine β2-M, and RBP levels, and estimated the glomerular filtration rate(e GFR) at the initiation of antiviral therapy and every 6 mo for a period of five years. RESULTS: Urine β2-M abnormalities were observed in patients during the first(n = 3), second(n = 7), third(n = 11), fourth(n = 16), and fifth(n = 21) year of ADV treatment. Urinary RBP abnormalities were observed in patients during the first(n = 2), second(n = 8), third(n = 12), fourth(n = 15), and fifth(n = 22) year of ADV treatment. e GFR decreased 20%-30% from baseline in 20 patients, 30%-50% in 12 patients, and > 50% in 3 patients during the five years of treatment. Further analysis indicated that decreases in e GFR of ≥ 30% relative to the baseline level correlated significantly with urine RBP and β2-M abnormalities. In contrast, both serum creatinine and e GFR remained stable in patients treated with entecavir, and only one of these patients developed a urine β2-M abnormality, and two developed urine RBP abnormalities during the five years of treatment. 3Infection with novel coronavirus(SARS-CoV-2)causes pneumonia in Rhesus macaques显示文摘The 2019 novel coronavirus(SARS-CoV-2)outbreak is a major challenge for public health.SARS-CoV-2 infection in human has a broad clinical spectrum ranging from mild to severe cases,with a mortality rate of^6.4%worldwide(based on World Health Organization daily situation report).However,the dynamics of viral infection,replication and shedding are poorly understood.Here,we show that Rhesus macaques are susceptible to the infection by SARS-CoV-2.After intratracheal inoculation,the first peak of viral RNA was observed in oropharyngeal swabs one day post infection(1 d.p.i.),mainly from the input of the inoculation,while the second peak occurred at 5 d.p.i.,which reflected on-site replication in the respiratory tract.Histopathological observation shows that SARS-CoV-2 infection can cause interstitial pneumonia in animals,characterized by hyperemia and edema,and infiltration of monocytes and lymphocytes in alveoli.We also identified SARS-CoV-2 RNA in respiratory tract tissues,including trachea,bronchus and lung;and viruses were also re-isolated from oropharyngeal swabs,bronchus and lung,respectively.Furthermore,we demonstrated that neutralizing antibodies generated from the primary infection could protect the Rhesus macaques from a second-round challenge by SARS-CoV-2.The non-human primate model that we established here provides a valuable platform to study SARS-CoV-2 pathogenesis and to evaluate candidate vaccines and therapeutics.Chao Shan Yan-Feng Yao Xing-Lou Yang Yi-Wu Zhou Ge Gao Yun Peng Lian Yang Xue Hu Jin Xiong Ren-Di Jiang Hua-Jun Zhang Xiao-Xiao Gao Cheng Peng Juan Min Ying Chen Hao-Rui Si Jia Wu Peng Zhou Yan-Yi Wang Hong-Ping Wei Wei Pang Zheng-Fei Hu Long-Bao Lv Yong-Tang Zheng Zheng-Li Shi Zhi-Ming Yuan 2020Cell Research2020,30,8:19

5Hepatitis B surface antigen levels during natural history of chronic hepatitis B: A Chinese perspective study显示文摘AIM: To determine the baseline hepatitis B surface antigen(HBsAg) levels during the different phases of chronic hepatitis B(CHB) patients in China.METHODS: Six hundred and twenty-three hepatitis B virus or un-infected patients not receiving antiviral therapy were analyzed in a cross-sectional study. The CHB patients were classified into five phases: immunetolerant(IT, n = 108), immune-clearance(IC, n = 161), hepatitis B e antigen negative hepatitis(ENH, n = 149), low-replicative(LR, n = 135), and liver cirrhosis(LC, n = 70). HBsAg was quantified(Abbott ARCHITECT assay) and correlated with hepatitis B virus(HBV) DNA, and serum alanine aminotransferase/aspartate aminotransferase(ALT/AST) in each phase of CHB was also determined.RESULTS: Median HBsAg titers were different in each phase of CHB(P < 0.001): IT(4.85 log10 IU/mL), IC(4.36 log10 IU/mL), ENH(2.95 log10 IU/mL), LR(3.18 log10 IU/mL) and LC(2.69 log10 IU/mL). HBsAg titers were highest in the IT phase and lowest in the LC phase. Serum HBsAg titers showed a strong correlation with HBV viral load in the IC phase(r = 0.683, P < 0.001). No correlation between serum HBsAg level and ALT/AST was observed.CONCLUSION: The mean baseline HBsAg levels differ significantly during the five phases of CHB, providing evidence on the natural history of HBV infection. 7Insight-HXMT observations of the first binary neutron star merger GW170817显示文摘Finding the electromagnetic(EM) counterpart of binary compact star merger, especially the binary neutron star(BNS) merger,is critically important for gravitational wave(GW) astronomy, cosmology and fundamental physics. On Aug. 17, 2017,Advanced LIGO and Fermi/GBM independently triggered the first BNS merger, GW170817, and its high energy EM counterpart,GRB 170817 A, respectively, resulting in a global observation campaign covering gamma-ray, X-ray, UV, optical, IR, radio as well as neutrinos. The High Energy X-ray telescope(HE) onboard Insight-HXMT(Hard X-ray Modulation Telescope) is the unique high-energy gamma-ray telescope that monitored the entire GW localization area and especially the optical counterpart(SSS17 a/AT2017 gfo) with very large collection area(~1000 cm^2) and microsecond time resolution in 0.2-5 MeV. In addition,Insight-HXMT quickly implemented a Target of Opportunity(ToO) observation to scan the GW localization area for potential X-ray emission from the GW source. Although Insight-HXMT did not detect any significant high energy(0.2-5 MeV) radiation from GW170817, its observation helped to confirm the unexpected weak and soft nature of GRB 170817 A. Meanwhile,Insight-HXMT/HE provides one of the most stringent constraints(~10^(-7) to 10^(-6) erg/cm^2/s) for both GRB170817 A and any other possible precursor or extended emissions in 0.2-5 MeV, which help us to better understand the properties of EM radiation from this BNS merger. However, these alloys suffer from serious biofouling, and may become vulnerable to corrosion attack under some extreme marine conditions. The passivating and biofouling performance of Ti alloys can be attributed to their compact,stable and protective films. This paper comprehensively reviews the passivating and biofouling behavior,as well as their mechanisms, for typical Ti alloys in various marine environments. 9Chinese herbal medicine reduces mortality in patients with severe and critical coronavirus disease 2019: a retrospective cohort study显示文摘This study aimed to evaluate the efficacy of Chinese herbal medicine(CHM)in patients with severe/critical coronavirus disease 2019(COVID-19).In this retrospective study,data were collected from 662 patients with severe/critical COVID-19 who were admitted to a designated hospital to treat patients with severe COVID-19 in Wuhan before March 20,2020.All patients were divided into an exposed group(CHM users)and a control group(non-users).After propensity score matching in a 1:1 ratio,156 CHM users were matched by propensity score to 156 non-users.No significant differences in seven baseline clinical variables were found between the two groups of patients.All-cause mortality was reported in 13 CHM users who died and 36 non-users who died.After multivariate adjustment,the mortality risk of CHM users was reduced by 82.2%(odds ratio 0.178,95%CI 0.076–0.418;P<0.001)compared with the non-users.Secondly,age(odds ratio 1.053,95%CI 1.023–1.084;P<0.001)and the proportion of severe/critical patients(odds ratio 0.063,95%CI 0.028–0.143;P<0.001)were the risk factors of mortality.These results show that the use of CHM may reduce the mortality of patients with severe/critical COVID-19.Guohua Chen Wen Su Jiayao Yang Dan Luo Ping Xia Wen Jia Xiuyang Li Chuan Wang Suping Lang Qingbin Meng Ying Zhang Yuhe Ke An Fan Shuo Yang Yujiao Zheng Xuepeng Fan Jie Qiao Fengmei Lian Li Wei Xiaolin Tong 2020Frontiers of Medicine2020,14,6:10
10Altered regional homogeneity in post-traumatic stress disorder: a resting-state functional magnetic resonance imaging study显示文摘Objective Little is known about the brain systems that contribute to vulnerability to post-traumatic stress disorder (PTSD). Comparison of the resting-state patterns of intrinsic functional synchronization, as measured by functional magnetic resonance imaging (fMRI), between groups with and without PTSD following a traumatic event can help identify the neural mechanisms of the disorder and targets for intervention. Methods Fifty-four PTSD patients and 72 matched traumatized subjects who experienced the 2008 Sichuan earthquake were imaged with blood oxygen level-dependent (BOLD) fMRI and analyzed using the measure of regional homogeneity (ReHo) during the resting state. Results PTSD patients presented enhanced ReHo in the left inferior parietal lobule and right superior frontal gyrus, and reduced ReHo in the right middle temporal gyrus and lingual gyrus, relative to traumatized individuals without PTSD. Conclusion Our findings showed that abnormal brain activity exists under resting conditions in PTSD patients who had been exposed to a major earthquake. 11Microbial D-amino acids and marine carbon storage显示文摘In nature, there are two conformational types of amino acids: L- and D-isomers. The L-amino acids are the predominant form and are used mainly for protein synthesis, while the D-amino acids are few in quantity but more diverse in terms of their biological functions. D-amino acids are produced by many marine microbes, which are important players in carbon and energy cycles in the ocean. As the major constituent of the marine organic carbon pool, D-amino acids can persist in the water column for a long time before being further transformed by chemical or biological processes or transported through physical processes(such as absorption and aggregation). 12Relations of Budd-Chiari syndrome to prothrombin gene mutation显示文摘BACKGROUND: Budd-Chiari syndrome (BCS) is a type of disease characterized by portal hypertension and/or hy- pertension of the inferior vena cava (IVC) due to the ob- struction of the hepatic veins (HV) and/or intrahepatic IVC outlet. Being etiologically complicated and obscure, BCS can be acquired or idiopathic and several gene muta- tions may be contributable. This study was to explore whether prothrombin gene mutation (F G20210A) takes part in the pathogenesis of BCS and to investigate their cor- relativity. METHODS: In 38 proven BCS patients and 70 controls, polymerase chain reaction-restriction fragment length poly- morphism (PCR-RFLP) was used to find F G20210A mutation. To detect whether there are any mutations, four steps were taken: purification of genome DNA from whole blood, amplification of special fragment by polymerase chain reaction, digestion of the fragment via restriction en- donuclease, and analysis of results by polyacrylamide gel electrophoresis. RESULTS: F G20210A mutation was not detected in all patients and controls. CONCLUSIONS: No F G20210A mutation exists in Chi- nese patients with BCS, nor correlativity between the oc- currence of BCS and F G20210A mutation. 15Resveratrol-loaded peptide-hydrogels inhibit scar formation in wound healing through suppressing inflammation显示文摘Scar formation seriously affects the repair of damaged skin especially in adults and the excessive inflammation has been considered as the reason.The self-assembled peptide-hydrogels are ideal biomaterials for skin wound healing due to their similar nanostructure to natural extracellular matrix,hydration environment and serving as drug delivery systems.In our study,resveratrol,a polyphenol compound with anti-inflammatory effect,is loaded into peptide-hydrogel(Fmoc-FFGGRGD)to form a wound dressing(Pep/RES).Resveratrol is slowly released from the hydrogel in situ,and the release amount is controlled by the loading amount.The in vitro cell experiments demonstrate that the Pep/RES has no cytotoxicity and can inhibit the production of pro-inflammatory cytokines of macrophages.The Pep/RES hydrogels are used as wound dressings in rat skin damage model.The results suggest that the Pep/RES dressing can accelerate wound healing rate,exhibit well-organized collagen deposition,reduce inflammation and eventually prevent scar formation.The Pep/RES hydrogels supply a potential product to develop new skin wound dressings for the therapy of skin damage.Chen-Chen Zhao Lian Zhu Zheng Wu Rui Yang Na Xu Liang Liang 2020Regenerative Biomaterials2020,7,1:8

17Progress in ENSO prediction and predictability study显示文摘ENSO is the strongest interannual signal in the global climate system with worldwide climatic, ecological and societal impacts. Over the past decades, the research about ENSO prediction and predictability has attracted broad attention. With the development of coupled models, the improvement in initialization schemes and the progress in theoretical studies, ENSO has become the most predictable climate mode at the time scales from months to seasons. This paper reviews in detail the progress in ENSO predictions and predictability studies achieved in recent years. An emphasis is placed on two fundamental issues: the improvement in practical prediction skills and progress in the theoretical study of the intrinsic predictability limit. 18Semi-supervised Ladder Networks for Speech Emotion Recognition显示文摘As a major component of speech signal processing, speech emotion recognition has become increasingly essential to understanding human communication. Benefitting from deep learning, many researchers have proposed various unsupervised models to extract effective emotional features and supervised models to train emotion recognition systems. In this paper, we utilize semi-supervised ladder networks for speech emotion recognition. The model is trained by minimizing the supervised loss and auxiliary unsupervised cost function. The addition of the unsupervised auxiliary task provides powerful discriminative representations of the input features, and is also regarded as the regularization of the emotional supervised task. We also compare the ladder network with other classical autoencoder structures. 20Copper single-atom catalysts with photothermal performance and enhanced nanozyme activity for bacteria-infected wound therapy显示文摘Nanozymes have become a new generation of antibiotics with exciting broad-spectrum antibacterial properties and negligible biological toxicity.However,their inherent low catalytic activity limits their antibacterial properties.Herein,Cu single-atom sites/N doped porous carbon(Cu SASs/NPC)is successfully constructed for photothermal-catalytic antibacterial treatment by a pyrolysis-etching-adsorption-pyrolysis(PEAP)strategy.Cu SASs/NPC have stronger peroxidase-like catalytic activity,glutathione(GSH)-depleting function,and photothermal property compared with non-Cu-doped NPC,indicating that Cu doping significantly improves the catalytic performance of nanozymes.Cu SASs/NPC can effectively induce peroxidase-like activity in the presence of H2O2,thereby generating a large amount of hydroxyl radicals(•OH),which have a certain killing effect on bacteria and make bacteria more susceptible to temperature.The introduction of near-infrared(NIR)light can generate hyperthermia to fight bacteria,and enhance the peroxidase-like catalytic activity,thereby generating additional•OH to destroy bacteria.Interestingly,Cu SASs/NPC can act as GSH peroxidase(GSH-Px)-like nanozymes,which can deplete GSH in bacteria,thereby significantly improving the sterilization effect.PTT-catalytic synergistic antibacterial strategy produces almost 100%antibacterial efficiency against Escherichia coli(E.coli)and methicillin-resistant Staphylococcus aureus(MRSA).In vivo experiments show a better PTT-catalytic synergistic therapeutic performance on MRSA-infected mouse wounds.Overall,our work highlights the wide antibacterial and anti-infective bio-applications of Cu single-atom-containing catalysts.Xianwen Wang Qianqian Shi Zhengbao Zha Dongdong Zhu Lirong Zheng Luoxiang Shi Xianwen Wei Lian Lian Konglin Wu Liang Cheng 2021Bioactive Materials2021,6,12:6
