| 1 | Efficacy and safety of metformin and sitagliptin based triple antihyperglycemic therapy(STRATEGY):a multicenter,randomized,controlled,non-inferiority clinical trial显示文摘Despite the current guideline's recommendation of a timely stepwise intensification therapy,the 'clinical inertia',termed as the delayed treatment intensification,commonly exists in the real world,which may be partly due to the relatively little substantial evidence and no clear consensus regarding the efficacy and safety of triple oral agents in patients inadequately controlled with dual therapy.In this clinical trial performed in 237 centers in China,5,535 type 2 diabetic patients inadequately controlled by previous therapies were treated with a stable metformin/sitagliptin dual therapy for 20 weeks.The patients who did not reach the glycated hemoglobin A1c(HbA1c) goal were then further randomized into glimepiride,gliclazide,repaglinide,or acarbose group for an additional 24-week triple therapy.A mean HbAlc reduction of 0.85%was observed when sitagliptin was added to the patients inadequately controlled with metformin in 16 weeks.Further HbAlc reductions in the 24-week triple therapy stage were 0.65%in glimepiride group,0.70%in gliclazide group,0.61%in repaglinide group,and 0.45%in acarbose group.The non-inferiority criterion for primary hypotheses was met for gliclazide and repaglinide,but not for acarbose,compared with glimepiride,when added to metformin/sitagliptin dual therapy.The incidences of adverse events(AEs) were 29.2%in the dual therapy stage and30.3%in the triple therapy stage.Metformin/sitagliptin as baseline therapy,with the addition of a third oral antihyperglycemic agent,including glimepiride,gliclazide,repaglinide,or acarbose,was effective,safe and well-tolerated for achieving an HbAlc<7.0%goal in type 2 diabetic patients inadequately controlled with previous therapies.The timely augmentation of up to three oral antihyperglycemic agents is valid and of important clinical benefit to prevent patients from exposure to unnecessarily prolonged hyperglycemia. | Wen Xu Yiming Mu Jiajun Zhao Dalong Zhu Qiuhe Ji Zhiguang Zhou Bin Yao Anhua Mao Samuel S.Engel Bin Zhao Yan Bi Longyi Zeng Xingwu Ran Juming Lu Linong Ji Wenying Yang Weiping Jia Jianping Weng | 2017 | Science China(Life Sciences)2017,60,3: | 19 |

| 4 | Activation of the Notch signaling pathway promotes neurovascular repair after traumatic brain injury显示文摘The Notch signaling pathway plays a key role in angiogenesis and endothelial cell formation, but it remains unclear whether it is involved in vascular repair by endothelial progenitor cells after traumatic brain injury. Therefore, in the present study, we controlled the Notch signaling pathway using overexpression and knockdown constructs. Activation of the Notch signaling pathway by Notch1 or Jagged1 overexpression enhanced the migration, invasiveness and angiogenic ability of endothelial progenitor cells. Suppression of the Notch signaling pathway with Notch1 or Jagged1 si RNAs reduced the migratory capacity, invasiveness and angiogenic ability of endothelial progenitor cells. Activation of the Notch signaling pathway in vivo in a rat model of mild traumatic brain injury promoted neurovascular repair. | 6 | Risks faced by donors of right lobe for living donor liver transplantation显示文摘BACKGROUND:Because of the shortage of deceased donors with livers fit for transplantation,living donor liver transplantation(LDLT)is becoming an attractive alternative.Attention should be paid to the donors,especially to those of the right lobe.In this study,we evaluated the risks faced by donors of the right lobe for adult-to-adult LDLT.METHODS:The perioperative data from 105 consecutive living donors of the right lobe performed in West China Hospital from January 2002 to December 2007 were retrospectively studied.Preoperative evaluation included CT,MRCP,and intraoperative cholangiography,showing liver volume,hepatic vasculature and the biliary system.The standard liver volume(SLV)and the ratio of left lobe volume to SLV were calculated.The right lobe grafts were obtained by transecting the liver on the right side of the middle hepatic vein without inflow vascular occlusion,using an ultrasonic dissector.After operation the donors were monitored in the Intensive Care Unit for about three days.Each donor was followed up for at least 6 months.RESULTS:There was no donor mortality.Major complications occurred in 14 donors(13.3%),of whom 3 received conservative treatment,8 required invasive paracentesis,and 3 required further surgery.All donors were recovered well and resumed their previous occupations.CONCLUSIONS:Donors of the right lobe face low risks.The preoperative evaluation,especially evaluation of the volume of the remnant liver,should be exact.During the operation,the patency of the remnant hepatic vasculature and bile duct must be preserved,and the extent of injury to the remnant liver should be limited as much as possible.The detection and treatment of postoperative complications should be diligently performed. | Ran, Shun Wen, Tian-Fu Yan, Lu-Nan Li, Bo Zeng, Yong Chen, Zhe-Yu Zhang, Yu Liao, Zhi-Xue Liang, Guan-Lin Li, Guo Zhang, Xian-Hua | 2009 | Hepatobiliary & Pancreatic Diseases International2009,8,6: | 8 |

Based on the parsing express of the guidance command change with varying time and adjoint system analysis method, the command characteristics and the non-dimensional miss distance of the guidance law are analyzed,a design principle of guidance order coefficients is discussed. Finally, based on the requirement of engineering, the method to calculate the guidance condition and maximal required acceleration of the guidance law is given. | 8 | Adaptive weighting impact angle optimal guidance law considering seeker's FOV angle constraints显示文摘In this paper, a new adaptive optimal guidance law with impact angle and seeker's field-of-view(FOV) angle constraints is proposed. To this end, the generalized optimal guidance law is derived first. A changeable impact angle weighting(IAW) coefficient is introduced and used to modify the guidance law to make it adaptive for all guidance constraints. After integrating the closed-form solution of the guidance command with linearized engagement kinematics, the analytic predictive models of impact angle and FOV angle are built, and the available range of IAW corresponding to constraints is certain. Next, a calculation scheme is presented to acquire the real-time value of IAW during the entire guidance process. | 12 | Comparison of three remotely sensed drought indices for assessing the impact of drought on winter wheat yield显示文摘Agricultural drought threatens food security.Numerous remote-sensing drought indices have been developed,but their different principles,assumptions and physical quantities make it necessary to compare their suitability for drought monitoring over large areas.Here,we analyzed the performance of three typical remote sensing-based drought indices for monitoring agricultural drought in two major agricultural production regions in Shaanxi and Henan provinces,northern China(predominantly rain-fed and irrigated agriculture,respectively):vegetation health index(VHI),temperature vegetation dryness index(TVDI)and drought severity index(DSI).We compared the agreement between these indices and the standardized precipitation index(SPI),soil moisture,winter wheat yield and National Meteorological Drought Monitoring(NMDM)maps.On average,DSI outperformed the other indices,with stronger correlations with SPI and soil moisture.DSI also corresponded better with soil moisture and NMDM maps.The jointing and grain-filling stages of winter wheat are more sensitive to water stress,indicating that winter wheat required more water during these stages.Moreover,the correlations between the drought indices and SPI,soil moisture,and winter wheat yield were generally stronger in Shaanxi province than in Henan province,suggesting that remote-sensing drought indices provide more accurate predictions of the impacts of drought in predominantly rain-fed agricultural areas. | Jianxi Huang Wen Zhuo Ying Li Ran Huang Fernando Sedano Wei Su Jinwei Dong Liyan Tian Yanbo Huang Dehai Zhu Xiaodong Zhang | 2020 | International Journal of Digital Earth2020,13,4: | 4 |

| 14 | Tenofovir vs.entecavir on recurrence of hepatitis B virus-related hepatocellular carcinoma beyond Milan criteria after hepatectomy显示文摘Background:Hepatectomy for hepatocellular carcinoma(HCC)beyond the Milan criteria is shown to be beneficial.However,a high rate of post-operative HCC recurrence hinders the long-term survival of the patients.This study aimed to investigate and compare the impacts of tenofovir(TDF)and entecavir(ETV)on the recurrence of hepatitis B viral(HBV)-related HCC beyond the Milan criteria.Methods:Data pertaining to 1532 patients who underwent hepatectomy and received antiviral therapy between January 2014 and January 2019 were collected from five centers.Recurrence-free survival(RFS)analysis was performed using the Kaplan-Meier method.Cox proportional hazards regression analysis was performed to determine prognostic factors for HCC recurrence.Results:The analysis incorporates 595 HBV-related HCC patients.The overall 5-year RFS was 21.3%.Among them,533 and 62 patients received ETV and TDF treatment,respectively.The 1-,3-,and 5-year RFS rates were 46.3%,27.4%,and 19.6%,respectively,in the ETV group compared with 65.1%,41.8%,and 37.2%,respectively,in the TDF group(P<0.001).Multivariate analysis showed that TDF treatment(hazard ratio[HR]:0.604,P=0.005),cirrhosis(HR:1.557,P=0.004),tumor size(HR:1.037,P=0.008),microvascular invasion(MVI)(HR:1.403,P=0.002),portal vein tumor thrombus(PVTT)(HR:1.358,P=0.012),capsular invasion(HR:1.228,P=0.040),and creatinine levels(CREA)(HR:0.993,P=0.031)were statistically significant prognostic factors associated with RFS.Conclusions:Patients with HCC beyond the Milan criteria exhibited a high rate of HCC recurrence after hepatectomy.Compared to the ETV therapy,TDF administration significantly lowered the risk of HCC recurrence. | Junyi Shen Weili Qi Junlong Dai Shusheng Leng Kangyi Jiang Yu Zhang Shun Ran Chuan Li Tianfu Wen | 2022 | Chinese Medical Journal2022,,3: | 3 |
