| 1 | 远端血管保护装置Guard Wire在急性心肌梗死急诊介入治疗中的应用显示文摘目的 评价GuardWire在急性心肌梗死 (AMI)急诊介入治疗时应用的价值。方法 连续观察 4 8例AMI急诊介入治疗并应用了GuardWire的患者 ,记录术中心电图变化及血流情况等 ,分析该装置的操作、造影和手术成功率 ,以及住院期间和术后 30d内的心血管事件发生率。结果 操作成功率、造影成功率和手术成功率均为 10 0 %。远端球囊阻塞时间平均 (8 1± 6 8)min ,所有患者均能良好耐受。 93 8%的患者吸引出肉眼可见的血栓 /斑块碎片 ,术中未出现因远端血管阻塞而引起的血液动力学或心电图、症状变化 ,未出现远端血管栓塞或无血流现象。住院期间和术后 30d内的心血管事件发生率均为 0。结论 该装置容易操作 ,可安全地应用于冠状动脉系统 ,有效防止AMI急诊介入治疗中可能出现的远端血管栓塞。 | 黄铮 陈红武 Osamu Katoh Shigeru Nakamura | 2003 | 中华心血管病杂志2003，31，11： | 14 |

| 3 | Protocol for laparoscopic cholecystectomy: Is it rocket science?显示文摘Laparoscopic cholecystectomy(LC) does not require advanced techniques, and its performance has therefore rapidly spread worldwide. However, the rate of biliary injuries has not decreased. The concept of the critical view of safety(CVS) was first documented two decades ago. Unexpected injuries are principally due to misidentification of human factors. The surgeon's assumption is a major cause of misidentification, and a high level of experience alone is not sufficient for successful LC. We herein describe tips and pitfalls of LC in detail and discuss various technical considerations.Finally, based on a review of important papers and our own experience, we summarize the following mandatory protocol for safe LC:(1) consideration that a high level of experience alone is not enough;(2) recognition of the plateau involving the common hepatic duct and hepatic hilum;(3) blunt dissection until CVS exposure;(4) Calot's triangle clearance in the overhead view;(5) Calot's triangle clearance in the view from underneath;(6) dissection of the posterior right side of Calot's triangle;(7) removal of the gallbladder body; and(8) positive CVS exposure. | 5 | Screening for hilar biliary invasion in ampullary cancer patients显示文摘BACKGROUND The treatment for ampullary cancer is pancreatoduodenectomy or local ampullectomy.However,effective methods for the preoperative investigation of hilar biliary invasion in ampullary cancer patients have not yet been identified.AIM To determine the necessity of and an appropriate method for investigating hilar biliary invasion of ampullary cancer.METHODS Among 43 ampullary cancer patients,34 underwent endoscopic treatment(n=9)or surgery(n=25).The use of imaging findings(thickening and enhancement of the bile duct wall on contrast-enhanced computed tomography,irregularity on endoscopic retrograde cholangiography,thickening of the entire bile duct wall on intraductal ultrasonography(IDUS),and partial thickening of the bile duct wall on IDUS)and biliary biopsy results for diagnosing hilar biliary invasion of ampullary cancer was compared.RESULTS Hilar invasion was not observed in every patient.Among the patients who did not undergo biliary stent insertion,the combination of partial thickening of the bile duct wall on IDUS and biliary biopsy results showed the highest accuracy(100%)for diagnosing hilar biliary invasion.However,each imaging method and biliary biopsy yielded some false-positive results.CONCLUSION Although some false-positive results were obtained with each method,the combination of partial thickening of the bile duct wall on IDUS and biliary biopsy results was useful for diagnosing hilar biliary invasion of ampullary cancer.However,hilar invasion of ampullary cancer is rare;therefore,the investigation of hilar biliary invasion of ampullary cancer might be unnecessary. | Tadayuki Takagi Mitsuru Sugimoto Rei Suzuki Naoki Konno Hiroyuki Asama Yuki Sato Hiroki Irie Jun Nakamura Mika Takasumi Minami Hashimoto Tsunetaka Kato Ryoichiro Kobashi Takumi Yanagita Yuko Hashimoto Shigeru Marubashi Takuto Hikichi Hiromasa Ohira | 2022 | World Journal of Gastrointestinal Endoscopy2022，14，9： | 2 |

| 7 | 在有固执地正常的丙氨酸 aminotransferase 的丙肝病毒病人的 IL-28B 多型性和治疗反应显示文摘 AIM:To examine the association between the interleukin 28B(IL-28B)genotype and treatment response in hepatitis C virus(HCV)-infected patients with persistently normal alanine aminotransferase(PNALT).METHODS:We compared the treatment response of HCV-infected patients with PNALT to that of patients with non-PNALT.Between February 2010 and April2013,278 patients infected with HCV were enrolled in this study.All of the patients were treated with peginterferon-alpha 2a or 2b plus ribavirin.In addition,180μg of peginterferon alpha-2a or 1.5μg/kg peginterferon alpha-2b per week plus weight-based ribavirin(600-1000 mg/d)were typically administered for 24 wk to HCV genotype 2-infected patients or for 48-72 wk to HCV genotype 1-infected patients.In all of the patients,the IL-28B rs8099917 genotype was determined using a TaqMan single-nucleotide polymorphism assay.HCV RNA was measured using the COBAS TaqMan HCV test.RESULTS:Female patients were dominant in the PNALT group(P<0.0001).Among 72 HCV genotype 1-infected patients with PNALT,the early virologic response(EVR)rates(P<0.01)and the sustained virologic response(SVR)rates(P<0.01)were higher in patients with the IL-28B TT genotype than in those with the IL-28B TG/GG genotype.In HCV genotype 1-infected patients with PNALT,multivariate logistic-regression analysis showed that SVR was independently predicted by the IL-28B rs8099917 TT type(P<0.05)and having an EVR(P<0.01).The IL-28B rs8099917 TT genotype strongly correlated with treatment response in HCV genotype 1-infected Asian patients with PNALT.CONCLUSION:The IL-28B genotype may be useful for selecting HCV genotype 1-infected patients with PNALT who should receive interferon-based treatment. | Tatsuo Miyamura Tatsuo Kanda Masato Nakamura Xia Jiang Shuang Wu Shingo Nakamoto Shigeru Mikami Nobuo Takada Fumio Imazeki Osamu Yokosuka | 2013 | World Journal of Hepatology2013，5，11： | 2 |
