| 1 | Placebo responses in patients with gastrointestinal disorders显示文摘Over the last several years there has been a growing interest in placebo, not only as an inert control in clinical trials, but also in the placebo effect as a group effect as well as a reaction in individual subjects. Methodological factors such as regression to the mean and natural history of the disease play a role in the evaluation of a possible placebo effect. In this report, we discuss several factors including Pavlovian conditioning, beliefs outcome, expectations, and other factors as potential mediators of the placebo response. | 6 | Ageing affects chondroitin sulfates and their synthetic enzymes in the intervertebral disc显示文摘The depletion of chondroitin sulfates(CSs)within the intervertebral disc(IVD)during degenerative disc disease(DDD)results in a decrease in tissue hydration,a loss of fluid movement,cell apoptosis,a loss of nerve growth inhibition and ultimately,the loss of disc function.To date,little is known with regards to the structure and content of chondroitin sulfates(CSs)during IVD ageing.The behavior of glycosaminoglycans(GAGs),specifically CSs,as well as xylosyltransferase I(XT-I)and glucuronyltransferase I(GT-I),two key enzymes involved in CS synthesis as a primer of glycosaminoglycan(GAG)chain elongation and GAG synthesis in the nucleus pulposus(NP),respectively,were evaluated in a bovine ageing IVD model.Here,we showed significant changes in the composition of GAGs during the disc ageing process(6-month-old,2-year-old and 8-year-old IVDs representing the immature to mature skeleton).The CS quantity and composition of annulus fibrosus(AF)and NP were determined.The expression of both XT-I and GT-I was detected using immunohistochemistry.A significant decrease in GAGs was observed during the ageing process.CSs are affected at both the structural and quantitative levels with important changes in sulfation observed upon maturity,which correlated with a decrease in the expression of both XT-I and GT-I.A progressive switch of the sulfation profile was noted in both NP and AF tissues from 6 months to 8 years.These changes give an appreciation of the potential impact of CSs on the disc biology and the development of therapeutic approaches for disc regeneration and repair. | Estelle C Collin Oliver Carroll Michelle Kilcoyne Marianna Peroglio Eugene See Doris Hendig Mauro Alini Sibylle Grad Abhay Pandit | 2017 | Signal Transduction and Targeted Therapy2017,2,1: | 2 |

| 10 | Effect of cyclic mechanical loading on immunoinflammatory microenvironment in biofabricating hydroxyapatite scaffold for bone regeneration显示文摘It has been proven that the mechanical microenvironment can impact the differentiation of mesenchymal stem cells(MSCs).However,the effect of mechanical stimuli in biofabricating hydroxyapatite scaffolds on the inflammatory response of MSCs remains unclear.This study aimed to investigate the effect of mechanical loading on the inflammatory response of MSCs seeded on scaffolds.Cyclic mechanical loading was applied to biofabricate the cell-scaffold composite for 15 min/day over 7,14,or 21 days.At the predetermined time points,culture supernatant was collected for inflammatory mediator detection,and gene expression was analyzed by qRT-PCR.The results showed that the expression of inflammatory mediators(IL1B and IL8)was downregulated(p<0.05)and the expression of ALP(p<0.01)and COL1A1(p<0.05)was upregulated under mechanical loading.The cell-scaffold composites biofabricated with or without mechanical loading were freeze-dried to prepare extracellular matrix-based scaffolds(ECM-based scaffolds).Murine macrophages were seeded on the ECM-based scaffolds to evaluate their polarization.The ECM-based scaffolds that were biofabricated with mechanical loading before freeze-drying enhanced the expression of M2 polarization-related biomarkers(Arginase 1 and Mrc1,p<0.05)of macrophages in vitro and increased bone volume/total volume ratio in vivo.Overall,these findings demonstrated that mechanical loading could dually modulate the inflammatory responses and osteogenic differentiation of MSCs.Besides,the ECM-based scaffolds that were biofabricated with mechanical loading before freeze-drying facilitated the M2 polarization of macrophages in vitro and bone regeneration in vivo.Mechanical loading may be a promising biofabrication strategy for bone biomaterials. | Penghui Zhang Xizhe Liu Peng Guo Xianlong Li Zhongyuan He Zhen Li Martin J.Stoddart Sibylle Grad Wei Tian Dafu Chen Xuenong Zou Zhiyu Zhou Shaoyu Liu | 2021 | Bioactive Materials2021,6,10: | 2 |
| 11 | Extracellular vesicles as mediators of vascular inflammation in kidney disease显示文摘Vascular inflammation is a common cause of renal impairment and a major cause of morbidity and mortality of patients with kidney disease. Current studies consistently show an increase of extracellular vesicles(EVs) in acute vasculitis and in patients with atherosclerosis. Recent research has elucidated mechanisms that mediate vascular wall leukocyte accumulation and differentiation. This review addresses the role of EVs in this process. Part one of this review addresses functional roles of EVs in renal vasculitis. Most published data address anti-neutrophil cytoplasmic antibody(ANCA) associated vasculitis and indicate that the number of EVs, mostly of platelet origin, is increased in active disease. EVs generated from neutrophils by activation by ANCA can contribute to vessel damage. While EVs are also elevated in other types of autoimmune vasculitis with renal involvement such as systemic lupus erythematodes, functional consequences beyond intravascular thrombosis remain to be established. In typical hemolytic uremic syndrome secondary to infection with shiga toxin producing Escherichia coli, EV numbers are elevated and contribute to toxin distribution into the vascular wall. Part two addresses mechanisms how EVs modulate vascular inflammation in atherosclerosis, a process that is aggravated in uremia. Elevated numbers of circulating endothelial EVs were associated with atherosclerotic complications in a number of studies in patients with and without kidney disease. Uremic endothelial EVs are defective in induction of vascular relaxation. Neutrophil adhesion and transmigration and intravascular thrombus formation are critically modulated by EVs, a process that is amenable to therapeutic interventions. EVs can enhance monocyte adhesion to the endothelium and modulate macrophage differentiation and cytokine production with major influence on the local inflammatory milieu in the plaque. They significantly influence lipid phagocytosis and antigen presentation by mononuclear phagocytes. Finally, platelet, erythrocyte and monocyte EVs cooperate in shaping adaptive T cell immunity. | 12 | Secukinumab provided significant and sustained improvement in the signs and symptoms of ankylosing spondylitis:results from the 52-week,PhaseⅢChina-centric study,MEASURE 5显示文摘Background:Secukinumab demonstrated sustained efficacy in patients with ankylosing spondylitis(AS)through 5 years in pivotal Phase III studies.Here,we present efficacy and safety results(52-week)of secukinumab in patients with AS from the MEASURE 5 study.Methods:MEASURE 5 was a 52-week,Phase III,China-centric study.Eligible patients were randomly assigned(2:1)to receive subcutaneous secukinumab 150 mg or placebo weekly for the first five doses and then once every 4 weeks(q4w).All placebo patients switched to secukinumab 150 mg q4w starting at Week 16.Primary endpoint was Assessments of SpondyloArthritis international Society(ASAS)20 at Week 16.Randomization was stratified by region(China vs.non-China).Results:Of 458 patients(secukinumab 150 mg,N=305;placebo,N=153)randomized,327(71.4%)were from China and 131(28.6%)were not from China.Of these,97.7%and 97.4%patients completed Week 16 and 91.1%and 95.3%(placebo-secukinumab)patients completed Week 52 of treatment.The primary endpoint was met;secukinumab significantly improved ASAS20 response at Week 16 vs.placebo(58.4%vs.36.6%;P<0.0001);corresponding rate in the Chinese population was 56.0%vs.38.5%(P<0.01).All secondary efficacy endpoints significantly improved with secukinumab 150 mg in the overall population at Week 16;responses were maintained with a trend toward increased efficacy from Week 16 to 52.No new or unexpected safety signals were reported up to Week 52.Conclusions:Secukinumab 150 mg demonstrated rapid and significant improvement in signs and symptoms of AS.Secukinumab was well tolerated and the safety profile was consistent with previous reports.Efficacy and safety results were comparable between the overall and Chinese populations. | Feng Huang Fei Sun Wei-Guo Wan Li-Jun Wu Ling-Li Dong Xiao Zhang Tae-Hwan Kim Raj Sengupta Ladislav Senolt Yi Wang Hao-Min Qiu Brian Porter Sibylle Haemmerle | 2020 | Chinese Medical Journal2020,,21: | 2 |
