| 3 | Effectiveness of Different Waist Circumference Cut-off Values in Predicting Metabolic Syndrome Prevalence and Risk Factors in Adults in China显示文摘Objective To study the effectiveness of waist circumference cut-off values in predicting the prevalence of metabolic syndrome(MetS) and risk factors in adults in China. Methods A cross-sectional survey was condcuted in 14 provinces(autonomous region, municipality) in China. A total of 47 325 adults aged ≥20 years were selected by multistage stratified sampling, and questionnaire survey and physical and clinical examination were conducted among them. MetS was defined according to the International Diabetes Federation(IDF) criteria and modified IDF criteria.Results The age-standardized prevalence of MetS was 24.2%(22.1% in men and 25.8% in women) and 19.5%(22.1% in men and 18.0% in women) according to the IDF criteria and modified IDF criteria respectively. The age-standardized prevalence of pre-MetS was 8.1%(8.6% in men and 7.8% in women) according to the modified IDF criteria. The prevalence of MetS was higher in urban residents than rural residents and in northern China residents than in southern China residents. The prevalence of central obesity was about 30% in both men and women according to the ethnicity-specific cut-off values of waist circumference for central obesity(90 cm for men and 85 cm for women). Multivariate regression analysis revealed no significant difference in risk factors between the two MetS definitions. | 4 | Efficacy of Solitaire AB stent-release angioplasty in acute middle cerebral artery atherosclerosis obliterative cerebral infarction显示文摘BACKGROUND In both national and international studies,the safety and effectiveness of treatment with the Solitaire stent in patients with ischemic stroke caused by acute large vessel occlusion were good,and the disability rate was significantly reduced.However,there are currently only a few reports on the differences in endovascular treatment for different etiological classifications,especially in the anterior cranial circulation,aorta atherosclerotic stenosis,and acute thrombosis.AIM To investigate the efficacy of Solitaire AB stent-release angioplasty in patients with acute middle cerebral artery atherosclerosis obliterative cerebral infarction.METHODS Twenty-five patients with acute middle cerebral atherosclerosis obliterative cerebral infarction were retrospectively enrolled in this study from January 2017 to December 2019.The Solitaire AB stent was used to improve anterior blood flow to maintain modified cerebral infarction thrombolysis[modified thrombolysis in cerebral infarction(mTICI)]at the 2b/3 level or above,the stent was then unfolded and released.RESULTS All 25 patients underwent successful surgery,with an average recanalization time of 23 min.One patient died of cerebral hemorrhage and cerebral herniation after the operation.The National Institutes of Health Stroke Scale(NIHSS)scores immediately after surgery(7.5±5.6),at 24 h(5.5±5.6)and at 1 wk(3.6±6.7)compared with the preoperative NIHSS score(15.9±4.4),were significantly different(P<0.01).One case of restenosis was observed 3 mo after surgery(the stenosis rate was 50%without clinical symptoms),the modified Rankin scale scores were 0 points in 14 cases(56%),1 point in 4 cases(16%),2 points in 2 cases(8%),3 points in 3 cases(12%),4 points in 1 case(4%),and 6 points in 1 case(4%).CONCLUSION In acute middle cerebral artery atherosclerosis obliterative cerebral infarction,when the Solitaire AB stent is unfolded and the forward blood flow is maintained at mTICI level 2b/3 or higher,stent release may be a safe and effective treatment method;however,long-term observation and a larger sample size are required to verify these findings. | Xi-Feng Wang Ming Wang Gang Li Xue-Yu Xu Wei Shen Jing Liu Shuang-Shuang Xiao Jiang-Hong Zhou | 2021 | World Journal of Clinical Cases2021,9,19: | 14 |

| 7 | Management of granulomatous lobular mastitis: an international multidisciplinary consensus(2021 edition)显示文摘Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the evidence.Levels of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and pathology.The approach to treatment of GLM should be applied according to the different clinical stage of GLM. | 9 | Parameters inversion of high central core rockfill dams based on a novel genetic algorithm显示文摘Parameters identification of rockfill materials is a crucial issue for high rockfill dams. Because of the scale effect, random sampling and sample disturbance, it is difficult to obtain the actual mechanical properties of rockfill from laboratory tests. Parameters inversion based on in situ monitoring data has been proven to be an efficient method for identifying the exact parameters of the rockfill. In this paper, we propose a modified genetic algorithm to solve the high-dimension multimodal and nonlinear optimal parameters inversion problem. A novel crossover operator based on the sum of differences in gene fragments(So DX) is proposed, inspired by the cloning of superior genes in genetic engineering. The crossover points are selected according to the difference in the gene fragments, defining the adaptive length. The crossover operator increases the speed and accuracy of algorithm convergence by reducing the inbreeding and enhancing the global search capability of the genetic algorithm. This algorithm is compared with two existing crossover operators. The modified genetic algorithm is then used in combination with radial basis function neural networks(RBFNN) to perform the parameters back analysis of a high central earth core rockfill dam. The settlements simulated using the identified parameters show good agreement with the monitoring data, illustrating that the back analysis is reasonable and accurate. | 10 | A New Species of the Genus Xenophrys(Amphibia: Anura: Megophryidae) from Libo County, Guizhou, China显示文摘A new species of the genus Xenophrys is described from a karst cave environment of Libo County, Guizhou Province, Southern China. The new species, Xenophrys liboensis sp. nov., is distinguished from its congeners by a combination of the following characters: 1) tympanum distinct; 2) vomerine teeth present; 3) the length of lower arm and hand larger than the half of snout-vent; 4) heels overlapped slightly when the flexed legs are held at right angles to the body axis; 5) toe tips with rudimentary webs and without grooves; 6) dermal fringes moderate; 7) tubercles on the dorsum forming an X-shaped weak ridge; 8) horn-like tubercle at the edge of the upper eyelid distinct; 9) color of the iris in life is brown. In Bayesian phylogenetic analysis of 22 species of Xenophrys, all the individuals of X. liboensis sp. nov. clustered into a monophyletic clade with high posterior probabilities. In addition, the ranges of genetic divergences of X. liboensis sp. nov. with other species were interspecific rather than intraspecific. | 13 | Dynamic failure mode and energy-based identification method for a counter-bedding rock slope with weak intercalated layers显示文摘The dynamic failure mode and energybased identification method for a counter-bedding rock slope with weak intercalated layers are discussed in this paper using large scale shaking table test and the Hilbert-Huang Transform(HHT) marginal spectrum.The results show that variations in the peak values of marginal spectra can clearly indicate the process of dynamic damage development inside the model slope.The identification results of marginal spectra closely coincide with the monitoring results of slope face displacement in the test.When subjected to the earthquake excitation with 0.1 g and 0.2 g amplitudes,no seismic damage is observed in the model slope,while the peak values of marginal spectra increase linearly with increasing slope height.In the case of 0.3 g seismic excitation,dynamic damage occurs near the slope crest and some rock blocks fall off the slope crest.When the seismic excitation reaches 0.4 g,the dynamic damage inside the model slope extends to the part with relative height of 0.295-0.6,and minor horizontal cracks occur in the middle part of the model slope.When the seismic excitation reaches 0.6 g,the damage further extends to the slope toe,and the damage inside the model slope extends to the part with relative height below 0.295,and the upper part(near the relative height of 0.8) slides outwards.Longitudinal fissures appear in the slope face,which connect with horizontal cracks,the weak intercalated layers at middle slope height are extruded out and the slope crest breaks up.The marginal spectrum identification results demonstrate that the dynamic damage near the slope face is minor as compared with that inside the model slope.The dynamic failure mode of counter-bedding rock slope with weak intercalated layers is extrusion and sliding at the middle rock strata.The research results of this paper are meaningful for the further understanding of the dynamic failure mode of counter-bedding rock slope with weak intercalated layers. | FAN Gang ZHANG Jian-jing FU Xiao ZHOU Li-rong | 2016 | Journal of Mountain Science2016,13,12: | 7 |
| 14 | Compression-related EMIC waves drive relativistic electron precipitation显示文摘With coordinated observations of the NOAA 15 satellite and OUL magnetometer station in Finland, we report that the electromagnetic ion cyclotron(EMIC) waves which were stimulated by the compression of the magnetosphere drive relativistic electron precipitation in geoquiescence on 1 Jan 2007. After an enhancement of solar wind dynamic pressure(SWDP), a dayside Pc1 pulsation was observed by the OUL station. Such a Pc1 pulsation is caused by an EMIC wave which propagates from the generation source to lower altitudes. Simultaneously, the NOAA 15 satellite registered an enhancement of precipitating electron count rates with energies >3 Me V within the anisotropic zone of protons. This phenomenon is coincident with the quasi-linear theoretical calculation presented in this paper. Accompanying w ith the remarkable improvements in the ability of generating and measuring high-accuracy time-frequency signal,seeking for new time-transfer techniques betw een distant clocks w ith much further improved accuracy attracts attentions w orld-w idely. The time-transfer technique based on optical pulses has the highest precision presently,and the further improvement in the accuracy is heavily dependent on the time-domain properties of the pulse as w ell as the sensitivity of the applied measurement on the exchanged pulse. The application of optical frequency comb in time transfer for a precision up to femtosecond level are currently the focus of much interest,and has recently achieved many breakthroughs. Further investigations show that,utilizing quantum techniques,i.e. quantum measurement technique and quantum optical pulse source,can lead to a new limit on the measured timing information. Furthermore,it can be immune from atmospheric parameters,such as pressure,temperature,humidity and so on.Such quantum improvements on time-transfer have a bright prospect in the future applications requiring extremely high-accuracy timing and ranging. | 20 | Toll-Like Receptor 4 Deficiency Causes Reduced Exploratory Behavior in Mice Under Approach-Avoidance Conflict显示文摘Abnormal approach-avoidance behavior has been linked to deficits in the mesolimbic dopamine(DA)system of the brain. Recently, increasing evidence has indicated that toll-like receptor 4(TLR4), an important pattern-recognition receptor in the innate immune system,can be directly activated by substances of abuse, resulting in an increase of the extracellular DA level in the nucleus accumbens. We thus hypothesized that TLR4-dependent signaling might regulate approach-avoidance behavior. To test this hypothesis, we compared the novelty-seeking and social interaction behaviors of TLR4-deficient(TLR4^(-/-))and wild-type(WT) mice in an approach-avoidance conflict situation in which the positive motivation to explore a novel object or interact with an unfamiliar mouse was counteracted by the negative motivation to hide in exposed,large spaces. We found that TLR4^(-/-)mice exhibitedreduced novelty-seeking and social interaction in the large open spaces. In less stressful test apparatuses similar in size to the mouse cage, however, TLR4^(-/-)mice performed normally in both novelty-seeking and social interaction tests. The reduced exploratory behaviors under approachavoidance conflict were not due to a high anxiety level or an enhanced fear response in the TLR4^(-/-)mice, as these mice showed normal anxiety and fear responses in the open field and passive avoidance tests, respectively. Importantly,the novelty-seeking behavior in the large open field induced a higher level of c-Fos activation in the nucleus accumbens shell(NAc Sh) in TLR4^(-/-)mice than in WT mice. Partially inactivating the NAc Sh via infusion of GABA receptor agonists restored the novelty-seeking behavior of TLR4^(-/-)mice. These data suggested that TLR4 is crucial for positive motivational behavior under approach-avoidance conflict.
