1Role of gut microbiota on intestinal barrier function in acute pancreatitis显示文摘Acute pancreatitis(AP)is a common gastrointestinal disorder.Approximately15%-20%of patients develop severe AP.Systemic inflammatory response syndrome and multiple organ dysfunction syndrome may be caused by the massive release of inflammatory cytokines in the early stage of severe AP,followed by intestinal dysfunction and pancreatic necrosis in the later stage.A study showed that 59%of AP patients had associated intestinal barrier injury,with increased intestinal mucosal permeability,leading to intestinal bacterial translocation,pancreatic tissue necrosis and infection,and the occurrence of multiple organ dysfunction syndrome.However,the real effect of the gut microbiota and its metabolites on intestinal barrier function in AP remains unclear.This review summarizes the alterations in the intestinal flora and its metabolites during AP development and progression to unveil the mechanism of gut failure in AP.Xue-Yang Li Cong He Yin Zhu Nong-Hua Lu 2020World Journal of Gastroenterology2020,26,18:75

442,573 cases of hepatectomy in China: a multicenter retrospective investigation显示文摘Hepatectomy is currently routinely performed in most hospitals in China. China owns the largest population of liver diseases and the biggest number of liver resection cases. A nationwide multicenter retrospective investigation involving 112 hospitals was performed, and focused on liver resection for patients with hepatocellular carcinoma(HCC). 42,573 cases of hepatectomy were enrolled, and 18,275 valid cases of liver resection for HCC patients were selected for statistical analysis. The epidemiology of HCC, distribution of hepatectomy, postoperative complications and prognosis were finally analyzed. In the 18,275 HCC patients,81% had hepatitis B virus infection and 10% had hepatitis C virus infection. 38% of the HCC patients had normal Alphafetoprotein(AFP) level, and other 35% had an AFP level lower than 400 ng mL^(-1). In the study period, 97% of the hepatectomy for HCC were treated with open surgery, and 23.81% had vascular exclusion techniques. The operation time was(191.7±105.6) min,the blood loss was(546.0±562.8) m L, and blood transfusion was(543.0±1,035.2) m L. The median survival for HCC patients was 631 days, with 1-, 3-, and 5-year overall survival of 73.2%, 28.8% and 19.6%, respectively. 9Acupuncture's Role in Solving the Opioid Epidemic: Evidence, Cost-Effectiveness, and Care Availability for Acupuncture as a Primary, Non-Pharmacologic Method for Pain Relief and Management-White Paper 2017显示文摘The United States(U.S.) is facing a national opioid epidemic, and medical systems are in need of nonpharmacologic strategies that can be employed to decrease the public's opioid dependence. Acupuncture has emerged as a powerful, evidence-based, safe, cost-effective, and available treatment modality suitable to meeting this need. Acupuncture has been shown to be effective for the management of numerous types of pain conditions, and mechanisms of action for acupuncture have been described and are understandable from biomedical, physiologic perspectives. Further, acupuncture's cost-effectiveness can dramatically decrease health care expenditures, both from the standpoint of treating acute pain and through avoiding addiction to opioids that requires costly care, destroys quality of life, and can lead to fatal overdose. Numerous federal regulatory agencies have advised or mandated that healthcare systems and providers offer non-pharmacologic treatment options for pain. Acupuncture stands out as the most evidence-based, immediately available choice to fulfil these calls. Acupuncture can safely, easily, and cost-effectively be incorporated into hospital settings as diverse as the emergency department, labor and delivery suites, andneonatal intensive care units to treat a variety of commonly seen pain conditions. 10Denitrification Losses and N_2O Emissions from Nitrogen Fertilizer Applied to a Vegetable Field显示文摘A field experiment was conducted on Chinese cabbage (Brassica campestris L. ssp. pekinensis (Lour.) Olsson) in a Nanjing suburb in 2003. The experiment included 4 treatments in a randomized complete block design with 3 replicates: zero chemical fertilizer N (CK); urea at rates of 300 kg N ha-1 (U300) and 600 kg N ha-1 (U600), both as basal and two topdressings; and polymer-coated urea at a rate of 180 kg N ha-1 (PCU180) as a basal application. The acetylene inhibition technique was used to measure denitrification (N2+N2O) from intact soil cores and N2O emissions in the absence of acetylene. Results showed that compared to CK total denitrification losses were significantly greater (P≤0.05) in the PCU180, U300, and U600 treatments, while N2O emissions in the U300 and U600 treatments were significantly higher (P≤0.05) than CK. In the U300 and U600 treatments peaks of denitrification and N2O emission were usually observed after N application. In the polymer-coated urea treatment (PCU180) during the period 20 to 40 days after transplanting, higher denitrification rates and N2O fluxes occurred. Compared with urea, polymer-coated urea did not show any effect on reducing denitrification losses and N2O emissions in terms of percentage of applied N. As temperature gradually decreased from transplanting to harvest, denitrification rates and N2O emissions tended to decrease. 11Sulfur-containing amino acid methionine as the precursor of volatile organic sulfur compounds in algea-induced black bloom显示文摘After the application of methionine, a progressive and significant increase occurred in five volatile organic sulfur compounds (VOSCs): methanethiol (MeSH), dimethyl sulfide (DMS), dimethyl disulfide (DMDS), dimethyl trisulfide (DMTS) and dimethyl tetrasulfide (DMTeS). Even in the untreated control without a methionine addition, methionine and its catabolites (VOSCs, mainly DMDS) were found in considerable amounts that were high enough to account for the water's offensive odor. However, blackening only occurred in two methionine-amended treatments. The VOSCs production was observed to precede black color development, and the reaching of a peak value for total VOSCs was often followed by water blackening. The presence of glucose stimulated the degradation of methionine while postponing the occurrence of the black color and inhibiting the production of VOSCs. In addition, DMDS was found to be the most abundant species produced after the addition of methionine alone, and DMTeS appeared to be the most important compound produced after the addition of methionine+glucose. These results suggest that methionine acted as an important precursor of the VOSCs in lakes suffering from algea-induced black bloom. 12Chinese Trauma Surgeon Association for management guidelines of vacuum sealing drainage application in abdominal surgeries-Update and systematic review显示文摘Vacuum sealing drainage (VSD) is frequently used in abdominal surgeries. However, relevant guidelines are rare. Chin ese Trauma Surge on Associati on orga nized a committee composed of 28 experts across China in July 2017, aiming to provide an evidence-based recommendation for the application of VSD in abdominal surgeries.Eleven questions regarding the use of VSD in abdominal surgeries were addressed:(1) which type of materials should be respectively chosen for the intraperitoneal cavity, retroperitoneal cavity and superficial incisions?(2) Can VSD be preventively used for a high-risk abdominal incision w让h primary suture?(3) Can VSD be used in severely contaminated/infected abdominal surgical sites?(4) Can VSD be used for temporary abdominal cavity closure under some special conditions such as severe abdominal trauma, infection, liver transplantation and intra-abdominal volume increment in abdominal compartment syndrome?(5) Can VSD be used in abdominal organ inflammation, injury, or postoperative drainage?(6) Can VSD be used in the treatment of intestinal fistula and pancreatic fistula?(7) Can VSD be used in the treatment of intra-abdominal and extra-peritoneal abscess?(8) Can VSD be used in the treatment of abdominal wall wounds, wound cavity, and defects?(9) Does VSD in crease the risk of bleeding?(10) Does VSD increase the risk of intestinal wail injury?(11) Does VSD increase the risk of peritoneal adhesion? Focusing on these questions, evidence-based recommendations were given accordingly. VSD was strongly recommended regarding the questions 2-4. Weak recommendations were made regarding questions 1 and 5-11. 13Shunting branch of portal vein and stent position predict survival after transjugular intrahepatic portosystemic shunt显示文摘AIM:To evaluate the effect of the shunting branch of the portal vein(PV)(left or right)and the initial stent position(optimal or suboptimal)of a transjugular intrahepatic portosystemic shunt(TIPS).METHODS:We retrospectively reviewed 307 consecu5tive cirrhotic patients who underwent TIPS placement for variceal bleeding from March 2001 to July 2010 at our center.The left PV was used in 221 patients and the right PV in the remaining 86 patients.And,224 and83 patients have optimal stent position and sub-optimal stent positions,respectively.The patients were followed until October 2011 or their death.Hepatic encephalopathy,shunt dysfunction,and survival were evaluated as outcomes.The difference between the groups was compared by Kaplan-Meier analysis.A Cox regression model was employed to evaluate the predictors.RESULTS:Among the patients who underwent TIPS to the left PV,the risk of hepatic encephalopathy(P=0.002)and mortality were lower(P<0.001)compared to those to the right PV.Patients who underwent TIPS with optimal initial stent position had a higher primary patency(P<0.001)and better survival(P=0.006)than those with suboptimal initial stent position.The shunting branch of the portal vein and the initial stent position were independent predictors of hepatic encephalopathy and shunt dysfunction after TIPS,respectively.And,both were independent predictors of survival.CONCLUSION:TIPS placed to the left portal vein with optimal stent position may reduce the risk of hepatic encephalopathy and improve the primary patency rates,thereby prolonging survival.Ming Bai Chuang-Ye He Xing-Shun Qi Zhan-Xin Yin Jian-Hong Wang Wen-Gang Guo Jing Niu Jie-Lai Xia Zhuo-Li Zhang Andrew C Larson Kai-Chun Wu Dai-Ming Fan Guo-Hong Han 2014World Journal of Gastroenterology2014,20,3:26
14Furazolidone-based triple and quadruple eradication therapy for Helicobacter pylori infection显示文摘AIM: To evaluate the efficacy of furazolidone-based triple and quadruple therapy in eradicating Helicobacter pylori(H. pylori) in a multi-center randomized controlled trial.METHODS: A total of 720 H. pylori positive patients with duodenal ulcer disease were enrolled at 10 different hospitals in Jiangxi province in China. The patients were randomly assigned to four treatment groups as follows: patients in Groups 1 and 3 received rabeprazole(10 mg), amoxicillin(1000 mg) and furazolidone(100 mg) twice daily for 7 and 10 d, respectively; patients in Groups 2 and 4 received rabeprazole(10 mg), bismuth(220 mg), amoxicillin(1000 mg) and furazolidone(100 mg) twice daily for 7 and 10 d, respectively. The primary outcome measure was H. pylori eradication rate 4 wk after treatment by intention-to-treat and per protocol analysis, while the secondary outcome measures were symptom and sign changes at the end of treatment and 4 wk after the end of treatment, as well as the proportion of patients who developed adverse events.RESULTS: The demographic data of the four groups were not significantly different. Overall, 666 patients completed the scheme and were re-assessed with the 13C-urea breath test. The intention-to-treat analysis of the H. pylori eradication rates in Groups 1, 2, 3 and 4 were 74.44%, 82.78%, 78.89% and 86.11%, respectively. The H. pylori eradication rate in Group 4 was significantly higher than that in Group 1. According tothe per protocol analysis, the H. pylori eradication rates in Groups 1, 2, 3 and 4 were 81.21%, 89.22%, 85.54% and 92.26%, respectively. The H. pylori eradication rate in Group 4 was significantly higher than that in Group 1. The number of adverse events was 15(8.3%), 16(8.9%), 15(8.3%) and 17(9.4%) in Groups 1, 2, 3 and 4, respectively, including dizziness, vomiting, diarrhea, nausea, skin rash, itchy skin, and malaise. 15Effects of Rice Straw and Its Biochar Addition on Soil Labile Carbon and Soil Organic Carbon显示文摘Whether the biochar amendment could affect soil organic matter(SOM) turnover and hence soil carbon(C) stock remains poorly understood. Effects of the addition of 13C-labelled rice straw or its pyrolysed biochar at 250 or 350°C to a sugarcane soil(Ferrosol) on soil labile C(di ssolved organic C, DOC; microbial biomass C, MBC; and mineralizable C, MC) and soil organic C(SOC) were investigated after 112 d of laboratory incubation at 25°C. Four treatments were examined as(1) the control soil without amendment(Soil);(2) soil plus 13C-labelled rice straw(Soil+Straw);(3) soil plus 250°C biochar(Soil+B250) and(4) soil plus 350°C biochar(Soil+B350). Compared to un-pyrolysed straw, biochars generally had an increased aryl C, carboxyl C, C and nitrogen concentrations, a decreased O-alkyl C and C:N ratio, but similar alkyl C and ?13C(1 742-1 877 ‰). Among treatments, significant higher DOC, MBC and MC derived from the new C(straw or biochar) ranked as Soil+Straw>Soil+B250>Soil+B350, whilst significant higher SOC from the new C as Soil+B250>Soil+Straw≈Soil+B350. Compared to Soil, DOC and MBC derived from the native soil were decreased under straw or biochar addition, whilst MC from the native soil was increased under straw addition but decreased under biochar addition. Meanwhile, native SOC was similar among the treatments, irrespective of the straw or biochar addition. Compared to Soil, significant higher total DOC and total MBC were under Soil+Straw, but not under Soil+B250 and Soil+B350, whilst significant higher total MC and total SOC were under straw or biochar addition, except for MC under Soil+B350. 16Rice CYP703A3, a cytochrome P450 hydroxylase, is essential for development of anther cuticle and pollen exine显示文摘Anther cuticle and pollen exine act as protective envelopes for the male gametophyte or pollen grain, but the mechanism underlying the synthesis of these lipidic polymers remains unclear. Previously, a tapetum‐expressed CYP703A3, a putative cytochrome P450 fatty acid hydroxylase, was shown to be essential for male fertility in rice(Oryza sativa L.). However,the biochemical and biological roles of CYP703A3 has not been characterized. Here, we observed that cyp703a3‐2 caused by one base insertion in CYP703A3 displays defective pollen exine and anther epicuticular layer, which differs from Arabidopsis cyp703a2 in which only defective pollen exine occurs. Consistently, chemical composition assay showed that levels of cutin monomers and wax components were dramatically reduced in cyp703a3‐2 anthers. Unlike the wide range of substrates of Arabidopsis CYP703A2, CYP703A3 functions as an in‐chain hydroxylase only for a specific substrate, lauric acid, preferably generating 7‐hydroxylated lauric acid. Moreover, chromatin immunoprecipitation and expression analyses revealed that the expression of CYP703A3 is directly regulated by Tapetum Degeneration Retardation, a known regulator of tapetum PCD and pollen exine formation. 17Delayed sowing can increase lodging resistance while maintaining grain yield and nitrogen use efficiency in winter wheat显示文摘Lodging resistance of winter wheat(Trnticum aestivum L.) can be increased by late sowing.However, whether grain yield and nitrogen use efficiency(NUE) can be maintained with delayed sowing remains unknown. During the 2013-2014 and 2014-2015 growing seasons, two winter wheat cultivars were sown on three dates(early sowing on October 1, normal so,wing on October8, and late sowing on October 15) to investigate the responses of lodging resistance, grain yield,and NUE to sowing date. No significant differences in lodging resistance, grain yield, or NUE between early and normal sowing were observed. Averaging over the two cultivars and years,postponing the sowing date significantly increased lodging resistance by 53.6% and 49.6%compared with that following early and normal sowing, respectively. Lodging resistance was improved mainly through a reduction in the culm height at the center of gravity and an increase in the tensile strength of the base internode. Late sowing resulted in similar grain yield as well as kernel weight and number of kernels per square meter, compared to early and normal sowing.Averaging over the two cultivars and years, delayed sowing resulted in a reduction in nitrogen uptake efficiency(UPE) by 11.0% and 9.9% compared to early and normal sowing, respectively,owing to reduced root length density and dry matter accumulation before anthesis. An average increase in nitrogen utilization efficiency(UTE) of 12.9% and 11.2% compared to early and normal sowing, respectively, was observed with late sowing owing to a reduction in the grain nitrogen concentration. The increase in UTE offset the reduction in UPE, resulting in equal NUEs among all sowing dates. 20Effect of dietary soy oil, glucose, and glutamine on growth performance, amino acid profile, blood profile, immunity, and antioxidant capacity in weaned piglets显示文摘Weaning stress results in gastrointestinal dysfunction and depressed performance in pigs. This study aimed to investigate the effect of soy oil, glucose, and glutamine on the growth and health of weaned piglets. Compared with those in the glutamine group, piglets in the glucose and soy oil groups had greater average daily gain, average daily feed intake, and gain: feed ratio from day 0 to 14, and gain: feed ratio for the overall period. There were no differences with regard to serum amino acids among the three groups on day 14, except glycine and threonine. The serum concentration of histidine, serine, threonine, proline, and cysteine was the highest in the glutamine group, while the content of glycine and lysine in the soy oil group on day 28 was the highest among all groups. Piglets fed with glutamine had greater serum glucose and creatinine on day 14, high-density lipoprotein on day 28, and serum IgG and IgM on day 28. Piglets in the glutamine group demonstrated lower serum total superoxide dismutase on day 14 and 28; however, they demonstrated higher total superoxide dismutase and total antioxidant capacity in the duodenum and ileum on day 14.
