Clozapine is strongly associated with the risk of pneumonia and inflammation显示文摘Clinicians need to remember that(1)systemic inflammations can increase clozapine level;(2)clozapine,by itself, can cause inflammation, particularly during titration that is too rapid for that patient;(3)clozapine may increase the risk of infection;and(4)more specifically, clozapine may be particularly strongly associated with the risk of pneumonia.Pneumonia appears to be associated with high mortality in clozapine patients around the world.Clinicians who are alert to the risk of pneumonia in clozapine patients may significantly decrease mortality in clozapine patients. There is no data on COVID-19 infections in clozapine patients, but based on what we know about clozapine pharmacology,we can hypothesise that clozapine, possibly by impairing immunological mechanisms, may increase the risk of pneumonia in infected patients.More importantly, once fever and/or pneumonia develops, the clozapine dose should be cut in half to decrease the risk of clozapine intoxication. If there is any doubt that in spite of halving the dose there are still signs of clozapine intoxication, completely stopping clozapine may be indicated.Once the signs of inflammation and fever have disappeared, the clozapine dose can be increased to the prior dosage level. | Jose de Leon Can-Jun Ruan Helene Verdoux Chuanyue Wang | 2020 | General Psychiatry2020,33,2: | 1

A bioinformatic evaluation of potential allergenicity of 85 candidate genes in transgenic organisms显示文摘The potential of genetically modified organisms(GMOs) to increase allergenicity or predispositions to allergies has attracted much attention. An approach that will properly and holistically evaluate the allergenicity of GMOs is yet to be found.Here,85 transgenes that have been reported in both international and domestic studies during recent years are summarized case by case;49 of the transformed genes were from plant sources and 36 were from animal sources.EVALLERTM,a web server for the in silico assessment of potential protein allergenicity, was used to evaluate the potential of the transgenic proteins as allergens. The biomedical journals listed in Highwire(http://gffzzd58bc32f34884012hkuwcccvuobwv69n0.ffgz.tsg.suse.edu.cn/) were searched and reviewed to decipher whether any of the transformed genes were linked to allergenicity or human health. The EVALLER analysis identified 5 allergenic genes, whilst our literature review found 11 genes that were either related to allergic cases or to clinical adverse events;all 16 of these genes have been used in GMOs.The analysis pathway that we have developed can help guide the selection of genes to be used in genetic modification. The pathway also provides a paradigm for allergenicity analysis of transgene candidates. | ZOU ZeHong HE Ying RUAN Lin SUN BaoQing CHEN HuiFang CHEN De LIU ShiMing YANG XiaoGuang TAO AiLin | 2012 | Chinese Science Bulletin2012,57,15: | 1

Feasibility and physics potential of detecting ^(8)B solar neutrinos at JUNO显示文摘The Jiangmen Underground Neutrino Observatory(JUNO)features a 20 kt multi-purpose underground liquid scintillator sphere as its main detector. Some of JUNO's features make it an excellent location for^8B solar neutrino measurements, such as its low-energy threshold, high energy resolution compared with water Cherenkov detectors, and much larger target mass compared with previous liquid scintillator detectors. In this paper,we present a comprehensive assessment of JUNO's potential for detecting^8B solar neutrinos via the neutrino-electron elastic scattering process.A reduced 2 MeV threshold for the recoil electron energy is found to be achievable, assuming that the intrinsic radioactive background^(238)U and^(232)Th in the liquid scintillator can be controlled to 10^(-17)g/g.With ten years of data acquisition, approximately 60,000 signal and 30,000 background events are expected. This large sample will enable an examination of the distortion of the recoil electron spectrum that is dominated by the neutrino flavor transformation in the dense solar matter, which will shed new light on the inconsistency between the measured electron spectra and the predictions of the standard three-flavor neutrino oscillation framework.IfDelta m^(2)_(21)=4.8times10^(-5);(7.5times10^(-5))eV^(2),JUNO can provide evidence of neutrino oscillation in the Earth at approximately the 3sigma(2sigma)level by measuring the non-zero signal rate variation with respect to the solar zenith angle. Moreover,JUNO can simultaneously measureDelta m^2_(21)using^8B solar neutrinos to a precision of 20% or better, depending on the central value, and to sub-percent precision using reactor antineutrinos.A comparison of these two measurements from the same detector will help understand the current mild inconsistency between the value of Delta m^2_(21)reported by solar neutrino experiments and the KamLAND experiment. | 18 | An international guideline with six personalised titration schedules for preventing myocarditis and pneumonia associated with clozapine显示文摘White blood cell(WBC)monitoring has reduced clozapinetreated patient deaths associated with agranulocytosis to a rarity. However, clozapine protocols and package inserts worldwide provide no instructions for preventing myocarditis or pneumonia during clozapine titrations.Prescribers worldwide are largely unaware of that.Meanwhile,as they worry about agranulocytosis, their clozapine-treated patients are at risk of dying from pneumonia or myocarditis.Consequently,an international guideline with 104 authors from 50 countries/regions was recently published to provide personalised clozapine titration schedules for adult inpatients. This forum article reviews pneumonia and myocarditis occurring during clozapine titration,as well as the three most innovative aspects of this new guideline:(1)personalised titration,(2)C reactive protein(CRP)measures,and(3)dose predictions based on blood levels.Clozapine metabolism is influenced by 3 levels of complexity:(1)ancestry groups,(2)sex-smoking subgroups,and(3)presence/absence of poor metabolizer status. These 3 groups of variables should determine the maintenance dose and speed of clozapine titration;they are summarised in a table in the full-text.The international clozapine titration guideline recommends measuring CRP levels simultaneously with WBC,at baseline and weekly at least for the first 4 weeks of titration, the highest risk period for clozapine-induced myocarditis. | Jose de Leon Can-Jun Ruan Georgios Schoretsanitis Christopher Rohde Elif Anıl Yağcıoğlu Trino Baptista Oleg O Kirilochev Carlos De las Cuevas Christoph U Correll | 2022 | General Psychiatry2022,35,3: | 0
A novel supplement with yeast b-glucan, prebiotic, minerals and Silybum marianum synergistically modulates metabolic and inflammatory pathways and improves steatosis in obese mice显示文摘Objective: To evaluate the synergic effects of a novel oral supplement formulation, containing prebiotics, yeast b-glucans, minerals and silymarin(Silybum marianum), on lipid and glycidic metabolism, inflammatory and mitochondrial proteins of the liver, in control and high-fat diet-induced obese mice. Methods: After an acclimation period, 32 male C57 BL/6 mice were divided into the following groups:nonfat diet(NFD) vehicle, NFD supplemented, high-fat diet(HFD) vehicle and HFD supplemented. The vehicle and experimental formulation were administered orally by gavage once a day during the last four weeks of the diet(28 consecutive days). We then evaluated energy homeostasis, inflammation, and mitochondrial protein expression in these groups of mice. Results: After four weeks of supplementation, study groups experienced reduced glycemia, dyslipidemia, fat, and hepatic fibrosis levels. Additionally, proliferator-activated receptor-α, AMP-activated protein kinase-1α, peroxisome proliferator-activated receptor γ co-activator-1α, and mitochondrial transcription factor A expression levels were augmented;however, levels of inhibitor of nuclear factor-κB kinase subunit a and p65 nuclear factor-κB expression, and oxidative markers were reduced. Notably, the cortisol/C-reactive protein ratio, a well-characterized marker of the hypothalamic–pituitary–adrenal axis immune interface status, was found to be modulated by the supplement. Conclusion: We discovered that the novel supplement was able to modify different antioxidant, metabolic and inflammatory pathways, improving the energy homeostasis and inflammatory status, and consequently alleviated hepatic steatosis. | Victor Abou Nehmi Gilson Masahiro Murata Ruan Carlos Macêdo de Moraes Gabriely Cristina AlvesLima Danielle Araujo De Miranda Katrin Radloff Raquel Galvao Figuerêdo Costa Joyce de Cassia Rosade Jesus Jéssica Alves De Freitas Nayara Izabel Viana Ruan Pimenta Katia Ramos Moreira Leite JoséPinhata Otoch Ana Flávia Marcal Pessoa | 2021 | Journal of Integrative Medicine2021,19,5: | 0
Impacts of COVID-19 on children and adolescents:A systematic review analyzing its psychiatric effects显示文摘OBJECTIVE To summarize the most relevant data from a systematic review on the impact of COVID-19 on children and adolescents, particularly analyzing its psychiatric effects.METHODS This review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses(PRISMA)guidelines and included experimental studies(randomized-individually or pooled-and non-randomized controlled trials),observational studies with a group for internal comparison(cohort studies-prospective and retrospective-and case-control)and qualitative studies in the period from 2021 to 2022.RESULTS The search identified 325 articles;we removed 125 duplicates. We selected 200 manuscripts, chosen by title and selected abstracts. We excluded 50 records after screening titles and abstracts,as they did not meet the inclusion criteria. We retrieved 150 records selected for a full reading. We excluded 90 text articles and we selected 25 records for the(n)final.Limitations:Due to the short period of data collection, from 2021 to 2022,there is a possibility of lack of relevant studies related to the mental health care of children and adolescents. In addition, there is the possibility of publication bias, such as only significant findings being published.CONCLUSION The impact of COVID-19 on the mental health of children and adolescents is of great concern to child and youth psychiatry.Situations such as fear, anxiety, panic, depression, sleep and appetite disorders,as well as impairment in social interactions caused by psychic stress, are punctual markers of pain and psychic suffering, which have increasing impacts on the mental health panorama of children and adolescents globally, particularly in vulnerable and socially at-risk populations. | Italo Wanderson de Moura Gabriel Danielly Gonçalves Sombra Lima Jeully Pereira Pires Nélio Barreto Vieira Aloisio Antonio Gomes de Matos Brasil Yara Talita Gomes Pereira Erika Galvao de Oliveira HildsonLeandro de Menezes Nadia Nara Rolim Lima Alberto Olavo Advíncula Reis Ruan Neto Pereira Alves Uanderson Pereira da Silva Jucier Gonçalves Junior Modesto Leite Rolim-Neto | 2022 | World Journal of Psychiatry2022,12,11: | 0
