| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. | 4 | Hygroscopic growth of aerosol scattering coefficient:A comparative analysis between urban and suburban sites at winter in Beijing显示文摘A humidity controlled inlet system was developed to measure the hygroscopic growth of aerosol scattering coefficient in conjunction with nephelometry at an urban site of Chinese Academy of Meteorological Sciences (CAMS) in Beijing and a rural site at Shangdianzi Regional Background Air Pollution Monitoring Station (SDZ) outside Beijing during winter, from December 2005 to January 2006. Measurements were carried out at a wavelength of 525 nm with an Ecotech M9003 nephelometer. The hygroscopic growth function (or factor) of the aerosol scattering coefficient f(RH) increased continuously with increasing relative humidity (RH) and showed no obvious 'step-like' deliquescent behavior at both sites during the experiment. The average growth factor f(RH) at the SDZ site could reach 1.5 when RH increased from less than 40% to 92%, and to 2.1 at the CAMS site when RH increased from less than 40% to 93%. The average hygroscopic growth factor at a relative humidity of 80%, f(RH = 80 ± 1%), was found to be about 1.26 ± 0.15 at CAMS and 1.24 ± 0.11 at SDZ. Further analysis indicated that under relatively polluted conditions, the average hygroscopic growth factor was higher at the CAMS site than that at the SDZ site. However, under relatively clean air conditions, the difference between the two sites was small, showing a hygroscopic growth behavior similar to those of burning biomass or blowing dust. | 7 | Combined APACH Ⅱ score and arterial blood lactate clearance rate to predict the prognosis of ARDS patients显示文摘Objective:To explore the easily applicable indicators of practical value to evaluate the prognosis of acute respiratory distress syndrome(ARDS).Methods:Blood and biochemical tests and bloodgas analyses were performed upon entry into the ICUs,12 h,24 h,48 h and 72 h after that in 72 ARDS patients(who were admitted to the ICUs of our hospital from January 2000 to December 2009).Then APACHEⅡscores were achieved by combining relevant physiological parameters and laboratory results.Results:There was a statistical difference between the death group and survival group at different time points upon entering the ICUs in terms of APACHEⅡscore, alveolar-arterial oxygen difference and arterial blood lactate clearance rate.PaO2/FiO2 values were recorded to be statistically different between the death group and survival group 24 h,48 h and 72 h,respectively after entry into the ICUs.In addition,registered linear regression existed between APACHEⅡscore,alveolar-arterial oxygen difference or PaO2/FiO2 value and time. | 8 | YAP promotes osteogenesis and suppresses adipogenic differentiation by regulatingβ-catenin signaling显示文摘YAP(yes-associated protein) is a transcriptional factor that is negatively regulated by Hippo pathway, a conserved pathway for the development and size control of multiple organs. The exact function of YAP in bone homeostasis remains controversial. Here we provide evidence for YAP's function in promoting osteogenesis, suppressing adipogenesis, and thus maintaining bone homeostasis.YAP is selectively expressed in osteoblast(OB)-lineage cells. Conditionally knocking out Yap in the OB lineage in mice reduces cell proliferation and OB differentiation and increases adipocyte formation, resulting in a trabecular bone loss. Mechanistically, YAP interacts with β-catenin and is necessary for maintenance of nuclear β-catenin level and Wnt/β-catenin signaling. Expression of β-catenin in YAP-deficient BMSCs(bone marrow stromal cells) diminishes the osteogenesis deficit. | 12 | Effects of long-term straw return on soil organic carbon fractions and enzyme activities in a double-cropped rice paddy in South China显示文摘Long-term straw return is an important carbon source for improving soil organic carbon(SOC) stocks in croplands, and straw removal through burning is also a common practice in open fields in South China. However, the specific effects of long-term rice straw management on SOC fractions, the related enzyme activities and their relationships, and whether these effects differ between crop growing seasons remain unknown. Three treatments with equal nitrogen, phosphorus, and potassium nutrient inputs, including straw/ash and chemical nutrients, were established to compare the effects of straw removal(CK), straw return(SR), and straw burned return(SBR). Compared to CK, long-term SR tended to improve the yield of early season rice(P=0.057), and significantly increased total organic carbon(TOC) and microbial biomass carbon(MBC) in double-cropped rice paddies. While SBR had no effect on TOC, it decreased light fraction organic carbon(LFOC) in early rice and easily oxidizable organic carbon(EOC) in late rice, significantly increased dissolved organic carbon(DOC), and significantly decreased soil p H. These results showed that MBC was the most sensitive indicator for assessing changes of SOC in the double-cropped rice system due to long-term straw return. In addition, the different effects on SOC fraction sizes between SR and SBR were attributed to the divergent trends in most of the soil enzyme activities in the early and late rice that mainly altered DOC, while DOC was positively affected by β-xylosidase in both early and late rice. | 13 | Assessment of left ventricular systolic synchronicity by real-time three-dimensional echocardiography in patients with dilated cardiomyopathy显示文摘Background Recent advances in real-time three-dimensional echocardiography (RT3DE) offer the potential to assess the left ventricular (LV) dyssynchrony simultaneously by analyzing the 17 segments time-volume curves. The purpose of this study was to test the feasibility and accuracy of RT3DE for quantitative evaluation of left ventricular systolic synchronicity. Methods Twenty-four patients with dilated cardiomyopathy (DCM) and twenty-five healthy volunteers were enrolled in this study. Full volume RT3DE was performed by using Philips IE33 with X3-1 probe. The global and 17-segmental time-volume curves were obtained by the on-line Qlab software (version 4.2). The time to minimal systolic volume in each segment (Tmsv) was taken to derive the following indexes of systolic asynchrony: Tmsv 16-SD, Tmsv 16-Dif, Tmsv 12-SD, Tmsv 12-Dif, Tmsv 6-SD and Tmsv 6-Dif, which meant the standard deviation or the maximal difference of Tmsv among the 16, 12 and 6 segments of the left ventricle respectively. The software also provided with each of the above parameters as a percentage of the cardiac cycle. Results Tmsv 16-SD, Tmsv 12-SD and Tmsv 6-SD were all significantly larger in the DCM group than those of the control group [Tmsv 16-SD: (52.9±40.6) ms vs (8.8±6.2) ms; Tmsv 12-SD: (29.5±30.8) ms vs (6.9±4.0) ms; Tmsv 6-SD: (28.9±34.6) ms vs (7.0±4.7) ms, all P≤0.001]. Tmsv 16-Dif, Tmsv 12-Dif and Tmsv 6-Dif were also significantly larger in the DCM group. There were close negative relations between the LVEF determined by RT3DE and each of the indexes of systolic asynchrony, among which the indexes of Tmsv-16-SD% and Tmsv-16-Dif% correlated most closely (r=-0.703 and r =-0.701, respectively). The DCM patients had significantly larger EDV and ESV, with significantly reduced LVEF compared with the healthy subjects. | 15 | Outcomes of preoperative chemoradiotherapy followed by surgery in patients with unresectable locally advanced sigmoid colon cancer显示文摘Background: Complete resection of locally advanced sigmoid colon cancer(LASCC) is sometimes difficult. Patients with LASCC have a dismal prognosis and poor quality of life, which has encouraged the evaluation of alternative multimodality treatments. This prospective study aimed to assess the feasibility and efficacy of neoadjuvant chemora?diotherapy(neo CRT) followed by surgery as treatment of selected patients with unresectable LASCC.Methods: We studied the patients with unresectable LASCC who received neo CRT followed by surgery between October 2010 and December 2012. The neoadjuvant regimen consisted of external?beam radiotherapy to 50 Gy and capecitabine?based chemotherapy every 3 weeks. Surgery was scheduled 6–8 weeks after radiotherapy.Results: Twenty?one patients were included in this study. The median follow?up was 42 months(range, 17–57 months). All patients completed neo CRT and surgery. Resection with microscopically negative margins(R0 resection) was achieved in 20 patients(95.2%). Pathologic complete response was observed in 8 patients(38.1%). Multivisceral resection was necessary in only 7 patients(33.3%). Two patients(9.5%) experienced grade 2 postopera?tive complications. No patients died within 30 days after surgery. For 18 patients with pathologic M0(yp M0) disease, the cumulative probability of 3?year local recurrence?free survival, disease?free survival and overall survival was 100.0%, 88.9% and 100.0%, respectively. | 17 | Liver transplantation for patients with hepatolithiasis显示文摘Because of its complicated pathological features such as repeated cholangitis and multiple operations, hepatolithiasis is difficult to treat and often lead to portal hypertension, and liver failure. The aim of this study was to investigate the indications of orthotopic liver transplantation ( OLT) for patients with hepatolithiasis and the improvement of operative techniques and the treatment of postoperative complications. METHODS:The data of 4 patients with hepatolithiasis who had undergone OLT at our department in recent years were reviewed retrospectively. RESULTS:The 4 patients with hepatolithiasis complicated with secondary biliary cirrhosis in end-stage liver disease included 1 woman and 3 men. One patient underwent OLT using veno-venous bypass prior to the mobilization of the liver. Two patients were subjected to liver transplantation with improved piggyback technique without bypass. One patient received OLT without bypass. All patients were operated on successfully and recovered uneventfully. Patient 1 had bleeding from the digestive tract on the postoperative day 6. Patient 2 had hemorrhage from the digestive tract and a leakage of end-to-side intestinal anastomosis on the postoperative day 44. Patient 4 was reoperated on because of hemorrhage from the anastomotic stoma of the hepatic artery on the postoperative days 8 and 10. In the 4 patients the mean operative time was 7.9 hours and blood loss was 910 ml. Postoperative pathological analysis revealed biliary liver cirrhosis. Follow-up of all patients showed good conditions. CONCLUSIONS:Hepatolithiasis with secondary biliary liver cirrhosis in patients with end-stage liver disease is indicated for liver transplantation. | 18 | Prevalence,clinical characteristics,risk factors,and indicators for lean Chinese adults with nonalcoholic fatty liver disease显示文摘BACKGROUND Nonalcoholic fatty liver disease(NAFLD)is one of the most common chronic diseases in the world.Nowadays,the percentage of non-obese or lean patients with NAFLD is increasing.NAFLD in non-obese populations,especially the lean subgroup with a normal waist circumference(WC),might lead to more problems than obese individuals,as these individuals may not visit clinics for NAFLD diagnosis or ignore the diagnosis of NAFLD.If the precise characteristics of these populations,especially the lean subgroup,are identified,the clinicians would be able to provide more appropriate advice and treatment to these populations.AIM To investigate the prevalence,clinical characteristics,risk factors,and possible indicators for NAFLD in lean Chinese adults with a normal WC.METHODS People without diabetes mellitus or significant alcohol consumption who underwent routine health examinations were included.Their fatty liver index(FLI),abdominal ultrasonography results,and controlled attenuation parameter were all assessed.Genotyping for single-nucleotide polymorphisms associated with NAFLD was performed in another small group consisting of biopsy-proven NAFLD subjects and healthy controls.RESULTS A total of 2715 subjects who underwent routine health examinations were included in the study.Among 810 lean participants with a normal WC,142(17.5%)fulfilled the diagnostic criteria for NAFLD.Waist-height ratio,hemoglobin,platelets,and triglycerides were significant factors associated with the presence of NAFLD in these participants.The appropriate cut-off value of the FLI score in screening for NAFLD in the lean subjects with a normal WC was 25.15,which had a 77.8%sensitivity and 75.9%specificity.There was no significant difference in the single-nucleotide polymorphisms in the SIRT1,APOC3,PNPLA3,AGTR1,and PPARGC1A genes between lean subjects with and without NAFLD(P<0.05).CONCLUSION NAFLD is not uncommon in lean Chinese adults even with a normal WC.Metabolic factors,rather than genetic factors,may play important roles in the development of NAFLD in this population.A lower cut-off value of the FLI score in screening for NAFLD should be used for lean Chinese adults with a normal WC. | Jing Zeng Rui-Xu Yang Chao Sun Qin Pan Rui-Nan Zhang Guang-Yu Chen Ying Hu Jian-Gao Fan | 2020 | World Journal of Gastroenterology2020,26,15: | 12 |

| 20 | Improving grain yield, nitrogen use efficiency and radiation use efficiency by dense planting, with delayed and reduced nitrogen application, in double cropping rice in South China显示文摘Improving both grain yield and resource use efficiencies simultaneously is a major challenge in rice production.However,few studies have focused on integrating dense planting with delayed and reduced nitrogen application to enhance grain yield,nitrogen use efficiency (NUE) and radiation use efficiency (RUE) in rice (Oryza sativa L.) in the double rice cropping system in South China.A high-yielding indica hybrid rice cultivar (Yliangyou 143) was grown in field experiments in Guangxi,South China,with three cultivation managements:farmers’practice (FP),dense planting with equal N input and delayed N application (DPEN) and dense planting with reduced N input and delayed N application (DPRN).The grain yields of DPRN reached 10.6 and 9.78 t ha^(–1) in the early and late cropping seasons,respectively,which were significantly higher than the corresponding yields of FP by 23.9–29.9%.The grain yields in DPEN and DPRN were comparable.NUE in DPRN reached 65.2–72.9 kg kg^(–1),which was 61.2–74.1% higher than that in FP and 24.6–30.2% higher than that in DPEN.RUE in DPRN achieved 1.60–1.80 g MJ^(–1),which was 28.6–37.9% higher than that in FP.The productive tiller percentage in DPRN was 7.9–36.2% higher than that in DPEN.Increases in crop growth rate,leaf area duration,N uptake from panicle initiation to heading and enhancement of the apparent transformation ratio of dry weight from stems and leaf sheaths to panicles all contributed to higher grain yield and higher resource use efficiencies in DPRN.Correlation analysis revealed that the agronomic and physiological traits mentioned above were significantly and positively correlated with grain yield.Comparison trials carried out in Guangdong in 2018 and 2019 also showed that DPRN performed better than DPEN.We conclude that DPRN is a feasible approach for simultaneously increasing grain yield,NUE and RUE in the double rice cropping system in South China. | FU You-qiang ZHONG Xu-hua ZENG Jia-huan LIANG Kai-ming PAN Jun-feng XIN Ying-feng LIU Yan-zhuo HU Xiang-yu PENG Bi-lin CHEN Rong-bing HU Rui HUANG Nong-rong | 2021 | Journal of Integrative Agriculture2021,20,2: | 12 |
